摘 要:目的通过对某三甲综合医院2015—2019年医疗纠纷赔付案例的分析,探讨解决途径和防范对策。方法对233例医疗纠纷赔付的患者基本情况、纠纷数量、科室分布、投诉内容、解决途径、关键因素等进行分析。结果60岁以上、医保支付方式、已婚、本地城镇患者发生纠纷的数量明显高于其他组别。2018年赔付数量最多,为59例;2016年赔付占比最高,占30.67%。普外科、骨外科、神经外科是医疗纠纷赔付高发科室;诊疗效果尤其手术不当是发生医疗赔付最重要的原因;医院与当事人调解是最主要的纠纷解决途径。结论通过对医疗纠纷赔付案例的分析,探讨发生的原因,医院要提高医疗人员业务水平,建立医疗风险防范干预机制和多元风险承担机制。Objective Through analysis on compensation cases of medical disputes from 2015 to 2019 in a tertiary general hospital,this paper summarizes key factors that cause medical disputes and explores solutions and countermeasures to avoiding them.Methods Statistical analyses were conducted on patient status,number of medical disputes,department distribution,complaint content,solutions and key factors of the 233 compensation cases of medical disputes.Results Medical disputes happened most frequently among local urban patients who are aged over 60,married and with medical insurance,with a rate apparently higher than any other categories.Most cases occurred in 2018,with the number of 59.Most compensations were made in 2016,with the rate of 30.67%.Compensation cases occurrence ranked the highest in the departments of general surgery,orthopedics and neurosurgery.Poor effectiveness,especially improper operation,was the major cause of medical disputes,while mediation between hospital and clients was the main resolution.Conclusion According to the analysis on the compensation cases of medical disputes and relevant causes,it is proposed that the hospital should improve medical staff's professional competence,establish medical risks prevention and intervention mechanism,and develop multiple risk-bearing mechanisms.
