希望与失望学说理论联合中医三联护理对糖尿病足坏疽截肢患者自我防护意识、希望水平及生活质量的影响观察

摘　　要：目的:研究并观察希望与失望学说理论联合中医三联护理对糖尿病足坏疽截肢患者自我防护意识、希望水平及生活质量的影响。方法:选择2016年10月~2021年1月于本院就诊的82例糖尿病足坏疽截肢患者,按随机数表法分为观察组和对照组,各41例。对照组接受常规护理干预,观察组同时应用希望与失望学说理论联合中医三联护理干预。观察两组术后并发症发生率、自我防护意识、负性情绪变化、希望水平以及生活质量评分变化。结果:观察组术后并发症发生率低于对照组(P<0.05);干预3个月后,观察组自我防护意识评分、希望评分、生活质量评分均高于对照组(P<0.05);观察组焦虑、抑郁评分均低于对照组(P<0.05)。结论:应用希望与失望学说理论联合中医三联护理对糖尿病足坏疽截肢患者进行干预,可降低术后并发症发生率,提升患者的自我防护意识和希望水平,减轻负性情绪影响,显著改善生活质量。Objective: To study and observe the effect of hope and disappointment theory combined with TCM triple nursing on self-protection consciousness, hope level and quality of life for patients with diabetic foot gangrene amputation.Methods: A total of 82 patients with diabetic foot gangrene amputation treated in the hospital were enrolled between October 2016 and January 2021.They were divided into an observation group and a controlled group according to random number table method, 41 cases in each group.The controlled group was given routine nursing, while the observation group was additionally given hope and disappointment theory combined with TCM triple nursing.The incidence of postoperative complications, changes in self-protection consciousness, negative emotions, hope level and quality of life scores in both groups were observed.Results: The incidence of postoperative complications in the observation group was lower than that in the controlled group(P<0.05).After 3 months of intervention, scores of self-protection consciousness, hope level and quality of life in the observation group were higher than those in the controlled group(P<0.05),and scores of anxiety and depression were lower than those in the controlled group(P<0.05).Conclusion: The application of hope and disappointment theory combined with TCM triple nursing for patients with diabetic foot gangrene amputation can reduce the incidence of postoperative complications, improve self-protection consciousness and hope level, reduce the effects of negative emotions and significantly improve quality of life.
