摘  要:目的探讨精神应激状态下正常血压水平健康男性飞行学员大脑中动脉血流动力学参数变化情况及其应用价值。方法采用标准心算模拟精神应激状态,选取健康男性飞行学员35例,经颅彩色多普勒超声(TCCD)检测大脑中动脉(MCA)应激前、后状态下血流动力学参数的变化,测量并记录收缩期峰值流速(Vs)、舒张末期流速(Vd)、平均血流速度(Vm)、搏动指数(PI)、阻力指数(RI),同时测量应激前、后心率和血压指标。结果精神应激状态下男性飞行学员大脑中动脉的Vs、Vd、Vm相比静息状态下明显增快,PI、RI则降低,各项血流动力学指标差异均具有统计学意义(P<0.05)。结论精神应激状态下,健康男性军事飞行学员的大脑中动脉血流速度明显增快,血管阻力降低,顺应性增加,具有较好的脑血流自我调节功能。Objective To explore the changes of blood flow mechanics parameters of middle cerebral artery in healthy male flight cadets under mental stress and its application value. Methods The mental stress state was simulated by standard mental arithmetic, and 35 healthy male cadets with normal blood pressure were selected. Transcranial color doppler ultrasound(TCCD) was used to detect the changes of hemodynamic parameters before and after middle cerebral artery(MCA) stress. The peak systolic velocity(Vs), end-diastolic velocity(Vd), mean blood flow velocity(Vm), pulsatility index(PI), and resistance index(RI) were measured, and the heart rate and blood pressure also were recorded at the same time. Results Compared with hemodynamic parameters in resting state, Vs, Vd, and Vm of middle cerebral artery of male flight cadets in mental stress state were significantly increased, while PI and RI were decreased. These differences in hemodynamic indexes were statistically significant(P<0.05). Conclusions Under the condition of mental stress, the blood flow velocity of the middle cerebral artery and vascular compliance of flight cadets increases significantly, vascular resistance reduces, which show good autoregulation of cerebral blood flow.
