摘  要:目的研究无创心肌做功超声成像技术评估经皮冠状动脉介入术(PCI术)患者左心室收缩功能的价值。方法选择80例行PCI术的患者及80例同期健康体检者,前者设置为PCI组,后者设置为对照组,观察两组常规超声心动图参数与无创心肌做功参数,并比较PCI组术前、术后1个月、3个月的无创心肌做功参数及左室射血分数(LVEF)变化。结果 PCI组的LVEF、二尖瓣口舒张早期峰值流速(E)低于对照组,左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)高于对照组(P<0.05);PCI组的左心室整体有用功(GCW)、整体做功效率(GWI)、整体做功功率(GWE)低于对照组,整体无用功(GWW)、左心室整体收缩期峰值纵向应变(GLS)高于对照组(P<0.05);术后1个月PCI患者的无创心肌做功参数及LVEF与术前比较无明显变化(P>0.05),术后3个月无创心肌做功参数及LVEF优于术前(P<0.05)。结论 PCI术患者的超声心动图参数、心肌做功参数与健康者存在差异,通过无创心肌做功超声成像技术能够定量评估PCI术患者的心肌做功。Objective To investigate the value of noninvasive myocardial work ultrasonography imaging in evaluating left ventricular systolic function in patients undergoing percutaneous coronary intervention(PCI). Methods 80 patients after PCI in our hospital were selected as PCI group, and another 80 healthy people in the same period were selected as control group. Conventional echocardiographic parameters and noninvasive myocardial work parameters were compared between the two groups, and the changes of non-invasive myocardial work parameters and left ventricular ejection fraction(LVEF) before surgery, 1 month, and 3 months after surgery in the PCI group were compared.Results LVEF and E in PCI group were lower than those in control group, while LVEDD, LVESD, LVEDV, LVESV were higher than those in control group(all P<0.05). GCW, GWI and GWE in PCI group were lower than those in control group, while GWW and GLS were higher than those in control group(all P<0.05). Noninvasive myocardial work parameters and LVEF at 3 months after operation were better than those before operation(P<0.05). Conclusions The echocardiographic indexes and myocardial work indexes in PCI patients are significantly different from those in healthy people. The non-invasive myocardial work ultrasound imaging technology could quantitatively evaluate the myocardial work of PCI patients.
