剪切波弹性成像中SD及其他弹性参数在鉴别诊断浅表淋巴结病变良恶性中的价值探究

摘　　要：目的探讨弹性模量标准差(SD)及其他弹性参数鉴别诊断浅表淋巴结病变良恶性的临床价值。方法选取71例患者的73个淋巴结,分别对其进行剪切波弹性成像检查,记录各个淋巴结的弹性模量最大值、平均值、最小值及SD,以超声引导下穿刺活检或手术病理为金标准,比较良恶性浅表淋巴结病变的上述剪切波弹性指征的差异。结果淋巴结恶性病变组患者年龄(53.4±10.7)显著大于良性病变组(44.5±15.9)(P=0.009);良恶性浅表淋巴结病变的弹性模量最大值、弹性模量平均值、SD存在统计学意义(20.7±7.2 vs 16.9±5.1,13.6±5.1 vs 11.1±4.2,3.7±1.7 vs 2.6±1.1,P=0.007、0.022、0.002);年龄、弹性模量最大值、弹性模量平均值、SD对良恶性浅表淋巴结病变的鉴别诊断效能分别为0.654(0.527~0.780)、0.677(0.549~0.805)、0.671(0.543~0.798)、0.710(0.581~0.839),差异无统计学意义(P>0.05)。结论 SD及弹性模量最大值、弹性模量平均值对浅表淋巴结良恶性的鉴别诊断均有一定的价值。Objective To investigate the clinical value of differential diagnosis of superficial lymph node lesions with standard deviation(SD) of elastic modulus and other elastic parameters. Methods 73 lymph nodes from 71 patients who came to the ultrasound department of our hospital were selected, and shear wave elastography was performed on them to record the maximum, mean, minimum elastic modulus and SD of every superficial lymph node. The difference of the above-mentioned shear wave elasticity indications between benign and malignant superficial lymph node lesions was compared with ultrasound-guided biopsy or surgical pathology as the gold standard. Results The age of patients in the malignant lymph node lesions group(53.4±10.7) was significantly higher than that in the benign lesion group(44.5±15.9)(P=0.009).The maximum, mean elastic modulus and SD of benign and malignant superficial lymph node lesions(20.7±7.2 vs 16.9±5.1,13.6±5.1 vs 11.1±4.2,3.7±1.7 vs 2.6±1.1,P=0.007,0.022,0.002, respectively) was significantly different. The efficacy of age, maximum modulus of elasticity, mean modulus of elasticity, SD for the differential diagnosis of benign and malignant superficial lymph node lesions were 0.654(0.527-0.780), 0.677(0.549-0.805),0.671(0.543-0.798) and 0.710(0.581-0.839), respectively, with no significant difference(P>0.05). Conclusions SD, maximum elastic modulus and mean elastic modulus have certain value in differentiating benign from malignant superficial lymph nodes.
