摘 要:目的探讨超声妇科影像报告与数据系统(GI-RADS)分类法在小儿卵巢肿块良恶性鉴别中的应用价值。方法回顾性分析经手术和病理证实的58例患儿的59个卵巢肿块的超声图像,并进行GI-RADS分类。采用Kappa系数分析不同年资医师分类结果的一致性。以病理结果为金标准,绘制受试者工作特征曲线(ROC)计算GI-RADS对恶性肿块的诊断效能。结果不同年资医师应用GI-RADS对肿块分类的一致性好(Kappa=0.715,P<0.001)。GI-RADS诊断恶性肿瘤的曲线下面积(AUC)为0.959,灵敏度为100%、特异度为83.4%、阳性预测值为57.9%、阴性预测值为100%。结论超声GI-RADS分类法在小儿卵巢肿块的良恶性鉴别中有较高的诊断价值,且不同年资医师分类一致性好,值得推广应用。Objective To investigate the application value of gynecologic imaging reporting and data system(GI-RADS) in differentiating benign and malignant ovarian masses in children. Methods The ultrasound images of 59 ovarian masses in 58 children confirmed by surgery and pathology were retrospectively analyzed and classified by GI-RADS. Kappa coefficient was used to analyze the consistency of doctors with different ages in classification results. With pathological results as the gold standard, the receiver operating characteristic curve(ROC) was drawn to calculate the diagnostic efficacy of GI-RADS for malignant tumors. Results The consistency of the classification of masses using GI-RADS by different seniority doctors was good(Kappa=0.715, P<0.001). The area under curve(AUC) of GI-RADS in the diagnosis of malignant tumors was 0.959, sensitivity was 100%, specificity was 83.4%, positive predictive value was 57.9% and negative predictive value was 100%. Conclusions GI-RADS has high diagnostic value in the differential diagnosis of benign and malignant ovarian tumors in children, and the classification consistency of doctors with different ages is good, which is worthy of popularization and application.
