徒手旋转在头位难产产妇助产护理中的应用分析

摘　　要：目的了解徒手旋转在头位难产产妇助产护理中的应用效果。方法选择我院100例2017年4月至2019年5月头位难产产妇。随机分组,传统产程护理组采取传统产程护理,徒手旋转助产护理组则采取徒手旋转助产护理。比较两组阴道分娩率;第二产程的平均时间、新生儿神经行为测定评分;护理前后产妇焦虑自评量表评分值、抑郁自评量表评分值;新生儿窒息的出现率以及产后出血发生率。结果徒手旋转助产护理组有更高的阴道分娩率,P<0.05。护理前传统产程护理组、徒手旋转助产护理组产妇焦虑自评量表评分值、抑郁自评量表评分值相近,P>0.05;护理后徒手旋转助产护理组产妇焦虑自评量表评分值、抑郁自评量表评分低于传统产程护理组,P<0.05。徒手旋转助产护理组第二产程的平均时间、新生儿神经行为测定评分和传统产程护理组比较有优势,P<0.05。徒手旋转助产护理组新生儿窒息的出现率以及产后出血发生率低于传统产程护理组,P<0.05。结论徒手旋转助产护理对头位难产的效果确切,可缩短第二产程,有效减少产后出血,减轻产妇的焦虑程度和抑郁程度,改善新生儿神经行为,并提高自然分娩率。Objective To understand the application effect of free hand rotation in the midwifery nursing of the dystocia of the head.Methods One hundred cases were selected from April 2017 to May 2019.Random group,traditional birth care group to take the traditional birth care,free hand rotation midwifery care group to take free hand rotation midwifery care.Comparison of vaginal delivery rates between the two groups;the average time of the second birth and the score of neonatal neurobehavioral measurement;maternal anxiety score before and after nursing,depression score;The incidence of neonatal asphyxia and postpartum hemorrhage.Results There was a higher rate of vaginal delivery in the free hand rotation midwifery care group,P<0.05.The score values of maternal anxiety self-assessment scale and depression self-assessment scale were similar in the traditional birth care group and the free hand rotation midwifery care group before nursing,P>0.05.After nursing,the scores of the Self-rating Anxiety Scale and the Self-rating Depression Scale of the free hand rotation midwifery care group were lower than those in the traditional birth care group,P<0.05.The average time of the second labor course,the neonatal neurobehavioral score and the traditional birth care group in the free hand rotation midwifery care group had advantages,P<0.05.The incidence of neonatal asphyxia and postpartum hemorrhage was lower in the free hand rotation midwifery care group than in the traditional birth care group,P<0.05.Conclusion The effect of free hand rotation midwifery care in the primary dystocia is accurate,which can shorten the second birth,reduce postpartum hemorrhage,reduce the degree of anxiety and depression,improve neonatal neurobehavior,and increase the rate of natural childbirth.
