输尿管软镜碎石术前留置双J管期间应用银花泌炎灵疗效观察

摘　　要：目的探讨在输尿管软镜碎石术前留置双J管期间应用银花泌炎灵的临床疗效。方法选取2018年5月—2021年5月江西省人民医院收治的无尿路感染肾结石患者120例,按随机数字表法分为试验组和对照组。术前均留置Fr6双J管1周,试验组留置双J管期间口服银花泌炎灵,对照组未口服银花泌炎灵。比较2组患者术前尿白细胞>10个/HP率、术后全身炎症反应综合征(SIRS)发生率、手术时间、血红蛋白下降值、术后住院时间和结石清除率。结果除6例患者外,试验组55例和对照组59例均由同一主刀医师顺利完成输尿管软镜碎石术。2组术后均无严重并发症。试验组出现术前尿白细胞>10个/HP的例数为20例(36.4%),对照组为38例(64.4%),试验组明显少于对照组(P<0.05)。试验组术后出现2例(3.6%)SIRS,对照组出现9例(15.3%),2组比较差异有统计学意义(P<0.05)。试验组的术后住院时间为(2.3±0.6)d,短于对照组的(2.9±1.0)d,差异有统计学意义(P<0.05)。2组的手术时间、血红蛋白下降值、结石清除率比较差异均无统计学意义(P均>0.05)。结论在输尿管软镜碎石术前留置双J管期间应用银花泌炎灵安全有效,可降低尿中白细胞,减少术后感染和缩短住院时间。Objective It is to explore the clinical efficacy of Yinhua Miyanling during the indwelling of double J tube before flexible ureteroscopic lithotripsy.Methods A total of 120 patients with kidney stones without urinary tract infection who were admitted to Jiangxi Provincial People’s Hospital from May 2018 to May 2021 were selected and divided into a experimental group and a control group according to a random number table.The Fr6 double J tube was indwelled for 1 week before the operation.The experimental group was orally administered Yinhua Miyanling during the indwelling of the double J tube,and the control group was not orally administered Yinhua Miyanling.The preoperative urinary white blood cell>10/HP rate,postoperative systemic inflammatory response syndrome(SIRS)rate,operation time,hemoglobin reduction value,postoperative hospital stay and stone clearance rate were compared between the two groups.Results Except for 6 patients,55 cases in the experimental group and 59 cases in the control group were successfully completed by the same surgeon.There were no serious complications in the two groups.There were 20 cases(36.4%)with preoperative urine leukocytes>10/HP in the experimental group and 38 cases(64.4%)in the control group,the experimental group was significantly less than the control group(P<0.05).There were 2 cases(3.6%)of SIRS in the experimental group and 9 cases(15.3%)in the control group after surgery,the difference between the two groups was statistically significant(P<0.05).The postoperative hospital stay in the experimental group was(2.3±0.6)days,which was shorter than the control group(2.9±1.0)days,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the operation time,hemoglobin reduction value,and stone clearance rate between the two groups(all P>0.05).Conclusion The application of Yinhua Miyanling during the indwelling of the double J tube before flexible ureteroscopic lithotripsy is safe and effective,which can reduce white blood cel
