磁控胶囊内镜在不明原因慢性腹痛诊断中的应用价值

摘　　要：目的:探讨磁控胶囊内镜(capsule endoscopy,CE)对不明原因慢性腹痛(unidentified chronic abdominal pain,UCAP)的诊断能力和应用价值。方法:选取2017年1月—2020年12月在我科就诊的UCAP患者166例,所有患者均行磁控CE检查,分析其阳性检出结果及诊断情况。结果:166例患者均顺利完成上消化道检查,162例完成了全小肠检查,完成率为97.6%;磁控CE共检出阳性病例137例,检出率为82.5%,其中小肠病变检出率为57.8%(96/166),上消化道病变检出率为50.6%(84/166),37.9%(63/166)的患者同时存在2种或以上病变;137例中共91例患者的慢性腹痛可由磁控CE检出结果解释病因,检出率为54.8%。结论:磁控CE对小肠疾病有较高的检出率,同时兼顾上消化道检查,安全、无创,对UCAP有较高的诊断率和应用价值。Objective To investigate the diagnostic value of NaviCam magnetic-controlled capsule endoscopy(NMCE)system in patients with unidentified chronic abdominal pain(UCAP).Methods 166 patients with UCAP who were treated in our department from January 2017 to December 2020 were selected.All patients underwent magnetic control CE examination,and their positive detection results and diagnosis were analyzed.Results According to analysis of 166 UCAP inpatients’NMCE examination materials,all patients finished examination of upper gastrointestinal tract.The completion rate for examination of small intestine was 97.6%.82.5% patients had the gastrointestinal lesions.Detection rates of small intestinal lesions and gastroesophageal lesions or both by NMCE were 57.8%,50.6% and 37.9%,respectively.The etiological diagnostic rate of NMCE for UCAP was 54.8%.Conclusion NMCE system is a useful tool for screening of UCAP with high detective rate and diagnostic rate.
