摘 要:目的对比分析多层螺旋CT(MSCT)、磁共振成像(MRI)诊断膝关节损伤的临床价值。方法回顾性分析67例经手术或关节镜证实的膝关节损伤患者临床资料,总结膝关节损伤的MRI及MSCT表现,并对比分析两种检查方法的诊断效能。结果67例患者,手术或关节镜证实半月板损伤35例(52.24%),韧带损伤37例(55.22%),骨损伤30例(44.78%)。MRI诊断半月板损伤的敏感度、特异度、准确度分别为91.43%、84.38%、88.06%,均明显高于MSCT(68.57%、62.50%、65.67%),差异有统计学意义(χ^(2)=5.714、3.925、9.443,P<0.05)。MRI诊断韧带损伤、骨损伤的准确度分别为76.12%、74.63%,与MSCT(73.13%、71.64%)对比均无统计学差异(χ^(2)=0.158、0.152,P>0.05)。结论MRI与MSCT均对膝关节损伤有重要诊断价值,其中前者对于半月板损伤的诊断效能较高。Objective To compare and analyze the clinical value of multi-slice spiral CT(MSCT)and magnetic resonance imaging(MRI)in the diagnosis of knee joint injury.Methods The clinical data of 67 patients with knee joint injury who were confirmed by operation or arthroscopy were retrospectively analyzed.The MRI and MSCT findings of knee joint injury were summarized.The diagnostic efficiency of the two methods was compared and analyzed.Results Of the 67 patients,there were 35(52.24%)cases with meniscus injury,37(55.22%)cases with ligament injury and 30(44.78%)cases with bone injury confirmed by operation or arthroscopy.The sensitivity,specificity and accuracy of MRI in the diagnosis of meniscus injury were 91.43%,84.38% and 88.06%,respectively,which were significantly higher than those of MSCT(68.57%,62.50% and 65.67%)(χ^(2)=5.714,3.925,9.443,P<0.05).The accuracies of MRI in diagnosis of ligament injury and bone injury were 76.12% and 74.63%,respectively.There was no significant difference in the above indexes compared with those of MSCT(73.13% and 71.64%)(χ^(2)=0.158,0.152,P>0.05).Conclusion Both MRI and MSCT are of important diagnostic value for knee joint injury.And the diagnostic efficiency of the former is higher for meniscus injury.
