摘 要:原发性失眠(primary insomnia,PI)是最常见的一种睡眠障碍。尽管我们对失眠的性质、病因和病理生理学的研究已在一定程度上取得进展,但目前仍然没有一种被普遍接受的病理生理学模型。现如今,最为广泛接受的假说认为PI是一种过度觉醒状态的表现。磁共振氢质子波谱(proton magnetic resonance spectroscopy,1H-MRS)提供了一种无创的方法来测量大脑中各神经递质与代谢产物的含量,新兴的多体素磁共振氢质子波谱甚至可以区别测量左右脑特定脑区域的神经递质与代谢产物浓度,这为PI的病理生理学、发病机制、诊断及治疗的研究打开一扇新的大门。本文系统总结了国内外以磁共振波谱为手段,研究原发性失眠各个脑区域不同神经递质与代谢产物的各项研究结果及其进展,为进一步探索PI的病理生理学机制及其诊断与治疗提供了新的视角。Primary insomnia(PI)is one of the most common sleep disorders.Although we have made some progress in the study of the nature,etiology and pathophysiology of insomnia,there is still no universally accepted pathophysiological model.Today,the most widely accepted hypothesis is that PI is a manifestation of an over-arousal state.Proton magnetic resonance spectroscopy(1H-MRS)provides a non-invasive method to measure the levels of various neurotransmitter and metabolites in the brain,emerging the multi-voxel magnetic resonance spectroscopy can even measure the concentration of neurotransmitters and metabolites in specific brain areas,for the primary insomnia of pathophysiology,pathogenesis,diagnosis and treatment of research provides a fresh view.This paper systematically summarized the domestic and foreign research results and development process of different neurotransmitters and metabolites in various brain regions of primary insomnia by means of magnetic resonance spectroscopy,providing a new perspective for further exploration of the pathophysiological mechanism,diagnosis and treatment of primary insomnia.
