从“亢害承制”论癌症“行气败乱”病机和肺癌治要

摘　　要：系统梳理“亢害承制”理论的《黄帝内经》渊源和历代名家的代表性注释,特别是清代医家虞抟关于“一元、六元和子来救母”等亢害承制元气论,表明元气是自然界万物化生和人体生命活动的原动力。重点论述了“行气败乱”癌症病机假说的理论内涵,即癌症的发生是由于脏腑虚弱,长期收受异常,造成五脏功能失调,五行生克制化失衡,导致机体脏腑组织形态结构变化异常、功能衰退的病理过程。并用“亢害承制”理论论述“行气败乱”癌症病机和补元治癌大法。脏腑虚弱是癌症发病的前提和基础,五脏功能失调、五行生克制化失衡是癌症发生的过程,“行气败乱”是癌症形成和发展的病理结果与状态。其中元气亏虚是癌症发生的关键并贯穿于整个病理过程。大补元气,恢复和促进机体的化生能力,改善虚弱脏腑功能,恢复五脏生克制化关系是癌症治疗的本质。肺癌的发生除了与先天一元亏虚有关外,还与其不胜之火亢、其子肾水寒救母不力密切相关,所以肺癌的治则包括大补元气、清火热、滋肾水。This paper systematically analyzes the origin of the theory of“harmful hyperactivity and responding inhibition”in Inner Canon of Huangdi and the representative annotations of famous scholars in past dynasties,especially YU Tuan’s theory of“primordial qi,six climatic qi and treating son element to save mother element”in the Qing Dynasty,which shows that the primordial qi is the driving force of all things in nature and human life activities.It focuses on the theoretical connotation of the cancer pathogenesis hypothesis of“elements and qi in disorder”,that is,the occurrence of cancer is due to the weakness of zang-fu organs and the abnormal long-term reception and storage,resulting in the dysfunction of five zang-organs,the restriction and generation imbalance of five elements and the abnormal changes in the morphological and structural changes of organs and tissues,and the pathological process of functional decline.The theory of“harmful hyperactivity and responding inhibition”is used to discuss cancer pathogenesis of elements and qi in disorder and cancer treatment method of tonifying primordial qi.The weakness of zang-fu organs is the premise and basis of cancer,the dysfunction of the five zang-organs and the restriction and generation imbalance of the five elements are the process of cancer,and elements and qi in disorder is the pathological result and state of cancer formation and development.Primordial qi deficiency is the key to the occurrence of cancer and runs through the whole pathological process.It is the essence of cancer treatment to replenish primordial qi,restore and promote the organism’s ability to regenerate,improve the function of weak zang-fu organs and restore the restriction and generation relationship between five zang-organs.In addition to the innate primordial qi deficiency,the occurrence of lung cancer is also related to hyperactive fire over restricting metal and kidney water(son)failing to save metal(mother).Therefore,the treatment principles of lung cancer include po
