“水土同治”针刺治疗男性不育症选穴研究概况

摘　　要：从“水土同治”理论出发,概述治疗男性不育症的辨证选穴规律。指出“水土同治”即“脾肾同治”,脾肾二脏与男性生殖密切相关。精血互生,先后天同调,则肾气旺盛,脾胃健运,精血充足。男性不育症的病因复杂,病机以脾肾亏虚为本,湿热、血瘀、肝郁为标。治疗以健脾补肾为根本,健脾亦是治标之关键。单用针刺或针刺联合中药治疗男性不育症常选取三阴交、足三里、肾俞、关元、气海、太溪等穴,并选用阴陵泉清热利湿,肝俞、太冲疏肝理气,血海、膈俞活血通络。现代研究发现针刺上述穴位能够调节下丘脑-垂体-睾丸性腺轴、改善氧化应激状况、增强机体免疫力、提升精子密度及活力而治疗男性不育症。参考文献31篇。Starting from the theory of“simultaneous treatment of water and earth”,this paper outlines the regularities of selecting acupoints based on syndrome differentiation for the treatment of male infertility.It is pointed out that“simultaneous treatment of water and earth”means“simultaneous treatment of spleen and kidney”,and the spleen and kidney are closely related to male reproduction.Essence and blood promote each other,treating the innate foundation and acquired foundation,so kidney qi is vigorous,the spleen and stomach are invigorated,the essence and blood are sufficient.The etiology of male infertility is complicated,and the pathogenesis is based on the deficiency of spleen and kidney as the root,and damp heat,blood stasis,and liver depression as the branch.Treatment is based on invigorating spleen and tonifying kidney,and invigorating spleen is also the key to treating symptoms.Acupuncture alone or acupuncture combined with traditional Chinese medicine is often used to treat male infertility.Sanyinjiao(SP6),Zusanli(S36),Shenshu(B23),Guanyuan(CV4),Qihai(CV6),Taixi(K3)and other points are often selected,and Yinlingquan(SP9)is used to clear heat and drain dampness,Ganshu(B18)and Taichong(Liv3)to soothe liver and regulate qi,Xuehai(SP10),Geshu(B17)to promote blood circulation and dredge collaterals.Modern studies have found that acupuncture at the above-mentioned acupoints can regulate the hypothalamic-pituitary-testicular gonad axis,improve oxidative stress,enhance immunity,increase sperm density and vitality,and treat male infertility.
