摘 要:目的探讨颅内动脉瘤3D打印模型在神经外科规范化培训中动脉瘤夹闭手术技能教学及培训的应用价值。方法选取就诊于哈尔滨医科大学附属第一医院神经外科的3名大脑中动脉动脉瘤患者,通过3D打印方法制作颅内动脉瘤模型,6位参培医生在经验丰富的教授讲解前后分别进行模拟夹闭操作,在手术体位、操作时间、夹闭准确度、载瘤动脉保护、手术器械使用、术前计划、显微镜使用、手术器械熟练程度方面进行评价,并比较讲解前后得分的差异。结果参培医生均成功完成模拟手术,各个项目的手术操作技能评分在模拟训练与教授讲解后均得到提高,讲解前后评价得分比较差异均有统计学意义(P<0.05)。结论在3D打印模型上进行颅内动脉瘤夹闭手术模拟训练可以提高接受住院医师规范化培训的医生的动脉瘤夹闭手术操作能力,此外,该模型还可以协助医生进行术前模拟并制定术前计划。Objective To explore the application value of the three-dimensional(3D)printed cerebral aneurysm model in teaching and training of aneurysm clipping surgery skills in the standardized training of neurosurgery.Methods Three middle cerebral artery aneurysm patients who were admitted to the Department of Neurosurgery of the First Affiliated Hospital of Harbin Medical University were selected,and 3 cases of middle cerebral artery intracranial aneurysm 3D printing models were made by 3D printing method.6 doctors who participated in the training performed simulated clipping operations before and after the explanation by the experienced professor.Evaluated in terms of surgical position,operation time,clipping accuracy,protection of the tumor-bearing artery,use of surgical instruments,preoperative planning,use of microscopes,and proficiency of surgical instruments.And compare the difference of the scores before and after the explanation and make a comprehensive evaluation and then compare the scores.Results The doctors who participated in the training successfully completed the simulated surgery.The scores of the surgical operation skills of each item were improved after the simulation training and the professor's explanation.The evaluation scores before and after the explanation were all significantly different(P<0.05).Conclusion Observation and simulation training of intracranial aneurysm clipping surgery on the 3D printed model can improve the skills of neurosurgery doctors who receive standardized training of residents for aneurysm clipping surgery.In addition,the model can also assist doctors in pre-operation Simulate and develop a preoperative plan.
