不同程度感音神经性听力损失儿童ASSR、click-ABR反应阈与行为听阈的相关性

摘　　要：目的探讨不同程度感音神经性听力损失儿童听性稳态反应(ASSR)、短声听性脑干反应(click-ABR)与行为阈值的相关性及在听障儿童听力评估中的价值。方法回顾性分析2019年1月至2019年12月确诊的46例(92耳)感音神经性听力损失儿童的ASSR、click-ABR及行为听阈结果,其中,轻度听力损失4耳,中度听力损失26耳,重度听力损失33耳,极重度听力损失29耳;分析不同程度听力损失者ASSR、click-ABR反应阈与行为听阈的相关性。结果0.5 kHzASSR反应阈与行为阈值的差值较1、2、4 kHz大,其中轻中度听力损失程度耳0.5 kHzASSR反应阈值与行为阈值的差值最大,为17.67±10.98 dB;不同程度听力损失组各频率ASSR反应阈与行为阈值均存在显著的相关性,相关系数均>0.7;轻中度、重度及极重度听力损失耳2、4 kHzASSR反应阈及行为听阈平均值与click-ABR存在显著的相关性,且ASSR反应阈与click-ABR反应阈相关系数(分别为0.810、0.851、0.870)大于行为阈值与click-ABR反应阈相关系数(分别为0.622、0.746、0.813)。结论本组不同程度感音神经性听力损失儿童ASSR反应阈与行为听阈有显著相关性,ASSR、click-ABR和行为测听联合应用,能更准确地评估婴幼儿听力。Objective To study the correlation between the auditory steady-state response(ASSR)test,click-evoked auditory brainstem response(click-ABR)and behavioral audiometry threshold in children with sensorineural hearing loss of different degrees,and to explore the value of these three tests in the hearing assessment of children with hearing impairment.Methods We retrospectively analyzed the hearing diagnosis results of 46 children(92 ears in total)who visited the clinic from January 2019 to December 2019.Statistical analysis was used to analyze the correlation between ASSR,click-ABR and behavioral audiometry threshold.Results The difference between the response threshold of ASSR 0.5 kHz and the behavioral audiometry threshold was larger than that of 1 kHz,2 kHz and 4 kHz,among which the degree of hearing loss was less than or equal to 60 dB.The maximum difference between the response threshold of ASSR 0.5 kHz and the behavioral audiometry threshold was 17.67±10.98 dB HL.There was a significant correlation between group ASSR and behavioral audiometry threshold.ASSR and behavioral audiometry threshold 2 kHz and 4 kHz response threshold average value had a significant correlation with click-ABR.The correlation coefficient between ASSR response threshold and click-ABR response threshold was greater than the correlation coefficient between behavior audiometry threshold and click-ABR response threshold.Conclusion There is a significant correlation between ASSR and behavioral audiometry.Combining ASSR,click-ABR and behavioral audiometry can more accurately assess infants’hearing level.
