酶联免疫吸附法和间接免疫荧光法检测大疱性类天疱疮抗体的比较

摘　　要：目的比较酶联免疫吸附法(ELISA)和间接免疫荧光法(IIF)检测大疱性类天疱疮(BP)的临床应用价值。方法选择89例BP患者(BP组)和同期排除BP的89例对照人群(对照组),采用间接免疫荧光法(IIF)检测抗表皮基底膜抗体,采用酶联免疫吸附法(ELISA)检测抗BP180和BP230抗体,比较两种检测指标的敏感性、特异性、阳性预测值(PPV)、阴性预测值(NPV)和诊断符合率。结果BP组和对照组中抗BP180抗体阳性率分别为94.38%(84/89)和17.98%(16/89),抗BP230抗体阳性率分别为58.43%(52/89)和7.87%(7/89),抗表皮基底膜抗体阳性率分别为82.02%(73/89)和0.00%(0/89)。在BP患者中抗BP180抗体的敏感度、特异度、PPV、NPV、诊断符合率分别为94.38%、82.02%、84.00%、93.59%、88.2%。抗BP230抗体分别为58.43%、92.13%、88.14%、68.91%、75.28%。抗表皮基底膜抗体抗体分别为82.02%、100.00%、100.00%、84.76%、91.01%。结论ELISA法检测抗BP180、BP230抗体与IIF法检测抗表皮基底膜抗体在BP的诊断中能相互补充,提高诊断阳性率。Objective To compare the effect of enzyme-linked immunosorbent assay(ELISA) method and indirect immunofluorescence(IIF) method in detecting the specific antibodies in the patients with bullous pemphigoid(BP).Methods 89 patients with BP was chosen as BP group.89 patients without BP were chosen as control group.Indirect immunofluorescence(IIF) was used to detect resistance to epidermal basement membrane antibodies,enzyme-linked immunosorbent assay(ELISA) was used to detect antibody BP180 and BP23 0.The sensitivity,specificity,positive predictive value and negative predictive value of each antibody between two methods were compared.Results Between the BP group and control group,the positive rates of anti-BP 180 antibody were 94.38%(84/89) and 17.98%(16/89),respectively.Anti-BP230 antibody were 58.43%(52/89) and 7.87%(7/89),respectively.Anti-epidermal basement membrane antibody were 82.02%(73/89) and 0.00%(0/89),respectively.In BP group,the sensitivity,specificity,positive predictive value(PPV) and negative predictive value(NPV) and diagnosis rates of anti-BP 180 antibody were 94.38%、82.02%、84.00%、93.59%、88.2%,respectively.Anti-BP230 antibody were 58.43%、92.13%、88.14%、68.91%、75.28% respectively in BP group.Anti-epidermal basement membrane antibody were 82.02%、100.00%、100.00%、84.76%、91.01% respectively in BP group.C onclusion ELISA and IIF are mutal complement and can increase the diagnostic positive rate they were used to detect BP 180 antibody,BP230 antibody and resistance to epidermal basement membrane antibodies.
