摘　　要：目的探讨应用封闭负压引流(vacuum sealing drainage,VSD)技术联合臀上动脉穿支皮瓣治疗骶尾部褥疮的临床效果。方法2016年1月至2019年1月,本院收治58例Ⅲ~Ⅳ度骶尾部褥疮患者。一期骶尾部褥疮行手术扩创,清除坏死组织及窦道后用持续封闭负压引流(VSD)进行治疗。二期用皮瓣移位修复。切取皮辩面积为8×6 cm~15×8 cm。结果56例(96.6%)患者骶尾部皮瓣成活良好,创面均一期愈合,6个月随访,褥疮均无复发。2例(3.4%)患者皮瓣远端切口裂开皮瓣下积液。结论封闭负压引流技术结合皮瓣治疗骶尾部褥疮疗效确切,可解决临床实际问题,远期效果好。Objective To observe the therapeutic effect of vaccum sealing drainage method combined with superior gluteal artery perforator flap transplantation to cure sacral tail pressure ulcer.Methods 58 patients withⅢ-Ⅳdegree sacral tail pressure ulcer admitted into our hospital from January 2016 to January 2019 were chosen as subjects.At the first stage,anticipatory debridement and removing necrosis tissue and sinus,VSD were used in the treatment.Then at the second stage,superior gluteal artery perforator flap was cut about 8 cm×6 cm to 15 cm×8 cm sizes to repair the wounds.Results 56 cases of skin flap were successful(accounting for96.6%),and those cases were followed up for 6 months to 1 years,there was no recurrence.2 cases of distant of skin flap were disruption of wound and subcutaneous effusion(accounting for 3.4%).Conclusion The technique of closed negative pressure drainage combined with skin flap is effective in the treatment of sacrall bedsore,which can solve the clinical problems and has a good long-term effect.
