摘　　要：目的基于META分析方法探讨人群HLA-DRB1基因多态性与结核病的关联性。方法计算机检索PubMed、MEDLINE、EMBASE、CNKI、万方等数据库(检索文献年限均为数据库建立至2021年5月),收集国内外各数据库中公开发表的关于结核病人群与HLA-DRB1关联的病例对照文献,对符合条件的研究结果用Stata 16.0软件进行META分析,采用95%可信区间(CI)和综合优势比(OR)评估HLA-DRB1等位基因与结核病风险之间的关联强度,Q检验和I^(2)统计量检验异质性,Egger和Begg检验用于评价发表偏倚。亚组分析基于种族和基因分型方法。结果共纳入病例对照研究28篇,包含肺结核组3808例和对照组5130例。病例组和对照组HLA-DRB1*0803、HLA-DRB1*1501和HLA-DRB1*1602的合并OR值及其95%CI分别为1.577(1.215~2.047)、1.347(1.001~1.811)、1.240(1.030~1.492)。敏感性分析提示结论稳定性好。结论HLA-DRB1*0803、HLA-DRB1*1501和HLA-DRB1*1602可能为结核病的易患基因。Objective To assess the association between HLA-DRB1 gene polymorphism with tuberculosis by meta-analysis.Methods PubMed,MEDLINE,EMBASE,CNKI,WANFANG databases were searched(the retrieval period was from the establishment of the database to May 2021).Publications associated with tuberculosis patients and HLA-DRB1 by case-control study in various databases at domestic and overseas were collected.All the relevant research results found in chosen articles were analyzed by Stata16 software.The correlation between HLA-DRB1 alleles and TB risk was assessed by 95%CI andOR.Qtest andI^(2) statistic were used to test the heterogeneity.Egger’s and Begg’s tests were used to evaluate publication bias.Subgroup analysis was based on race and genotyping.28 articles were brought into the comparative study including 3808 cases in tuberculosis group and 5130 cases in control group.Results The pooledOR and 95%CIof HLA-DRB1*0803,HLA-DRB1*1501 and HLA-DRB1*1602 in case group and control group were 1.577(1.215,2.047),1.347(1.001,1.811)and 1.240(1.030,1.492).Sensitivity analysis showed that the conclusion drawn from our paper was stable.Conclusion HLA-DRB1*0803,HLA-DRB1*1501 and HLA-DRB1*1602 may be susceptible genes for tuberculosis infection.
