微视野检查对糖尿病性黄斑水肿视功能的评估

摘　　要：目的分析微视野检查对糖尿病性黄斑水肿视功能评价的意义。方法回顾性分析郑州大学第一附属医院2019年10月至2020年9月糖尿病性视网膜病变38例(68眼)的临床资料。所有患者玻璃体内注射阿柏西普前行微视野检查,观察记录视敏度、2°及4°注视稳定性。术后1个月复查并对比观察指标。结果微视野检查显示各象限及平均视敏度与视力负相关(r=-0.612,P<0.001),与黄斑中心区厚度无相关性(P>0.05)。2°及4°注视稳定性与视力负相关(r=-0.436,-0.533;均P<0.001)。阿柏西普玻璃体内注射后,各象限视敏度、2°及4°注视稳定性均有所提高(t=3.69,5.83,4.73;P=0.005,0.006,0.004)。结论微视野检查可评估糖尿病性黄斑水肿,视敏度及注视稳定性。Objective To analyze the significance of visual function evaluation of diabetic macular edema(DME)with microperimetry.Methods The clinical data of 68 eyes of 38 patients with diabetic retinopathy from Oct.2019 to Sep.2020 in the First Affiliated Hospital of Zhengzhou University were analyzed retrospectively.All cases received microperimetry to observe and record the visual sensitivity,2°and 4°fixation stability before intravitreal injection of aflibercept.The above-mentioned examinations were retested at 1 month after surgery,and statistical analysis was performed.Results The microperimetry showed each quadrant and average visual sensitivity were negatively correlated with visual acuity(r=-0.612,P<0.001),irrelevant with CMTX(P>0.05).The 20 and 40 fixation stability were negatively correlated with BCVA(r=-0.436,-0.533;all P<0.001).After intravitreal injection of aflibercept,the visual sensitivity of each quadrant and the stability fixation of 2°and 4°were improved(t=3.690,5.830,4.730;P=0.005,0.006,0.004).Conclusion Microperimetry can assess DME visual sensitivity and fixation stability.
