双锁定钢板内固定术治疗成人肱骨髁间骨折的临床观察

摘　　要：目的分析成人肱骨髁间骨折的疾病特点,评价双锁定钢板内固定术治疗的预后效果。方法80例成人肱骨髁间骨折患者,随机分为观察组和对照组,每组40例。观察组采用双锁定钢板内固定术治疗,对照组采用常规固定治疗。比较两组患者手术效果、围术期指标(手术时间、出血量、术后住院时间)以及健康调查简表(SF-36)评分。结果观察组患者总有效率为97.50%,高于对照组的80.00%,差异具有统计学意义(P<0.05)。观察组患者术中出血量(50.50±8.50)ml少于对照组的(80.50±10.50)ml,切口长度(4.30±0.50)cm、术后住院时间(14.50±1.50)d短于对照组的(12.30±1.80)cm、(17.50±2.50)d,SF-36评分(90.05±4.50)分高于对照组的(82.30±3.30)分,差异具有统计学意义(P<0.05)。结论与常规固定治疗比较,双锁定钢板内固定术治疗成人肱骨髁间骨折预后效果良好,手术损伤小、术后恢复快,能够显著提升患者的生活质量。Objective To analyze the characteristics of adult humeral intercondylar fractures and evaluate the prognostic effect of double-locking plate internal fixation.Methods A total of 80 cases of adult patients with humeral intercondylar fractures were randomly divided into observation group and control group,with 40 cases in each group.The observation group was treated with double-locking plate internal fixation,and the control group was treated with conventional fixation.The surgical effects,perioperative indicators(operation time,blood loss,postoperative hospitalization time)and 36-item short-form health survey(SF-36)score were compared between the two groups.Results The total effective rate in the observation group was 97.50%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).The intraoperative blood loss(50.50±8.50)ml of the observation group was less than(80.50±10.50)ml of the control group;the incision length(4.30±0.50)cm and the postoperative hospitalization time(14.50±1.50)d of the observation group were shorter than(12.30±1.80)cm and(17.50±2.50)d of the control group;the SF-36 score(90.05±4.50)points was higher than(82.30±3.30)points of the control group;all the differences were statistically significant(P<0.05).Conclusion Compared with conventional fixation,double-locking plate internal fixation for the treatment of adult humeral intercondylar fractures has a good prognosis,with less surgical damage and faster postoperative recovery,which can significantly improve the quality of life of patients.
