左卡尼汀注射液与福辛普利钠片联合用药方案用于维持性血液透析的临床效果评价

摘　　要：目的探究左卡尼汀注射液与福辛普利钠片联合用药方案用于维持性血液透析的临床效果,为临床医师选择合理用药方案提供参考。方法346例受维持性血液透析患者,依据随机数字表法分为对照组和观察组,每组173例。对照组患者仅接受左卡尼汀注射液治疗,观察组患者采用左卡尼汀注射液与福辛普利钠片联合用药方案治疗。对比两组的临床治疗效果、相关实验室指标[血肌酐(SCr)、血红蛋白(Hb)、C反应蛋白(CRP)、白蛋白(Alb)、肿瘤坏死肿瘤因子-α(TNF-α)和白细胞介素-6(IL-6)]及不良反应发生情况。结果观察组治疗总有效率为98.27%,显著高于对照组的91.91%,差异有统计学意义(P<0.05)。观察组Hb(124.25±6.78)g/L、Alb(38.64±3.54)g/L均高于对照组的(112.36±7.45)、(34.32±2.67)g/L,SCr(104.56±75.39)μmol/L、CRP(5.31±0.97)mg/L、TNF-α(10.16±1.07)pg/ml和IL-6(8.68±1.28)pg/ml低于对照组的(130.53±76.68)μmol/L、(6.49±1.27)mg/L、(12.84±1.58)pg/ml、(14.13±2.41)pg/ml,差异有统计学意义(P<0.05)。观察组的不良反应发生率为1.16%,低于对照组的6.36%,差异有统计学意义(P<0.05)。结论左卡尼汀注射液与福辛普利钠片联合用药方案用于维持性血液透析的临床效果显著,有效减少不良反应发生。Objective To investigate the clinical effect of levocarnitine injection combined with fosinopril sodium tablets for maintenance hemodialysis,so as to provide reference for clinicians to choose rational drug use regimen.Methods A total of 346 patients undergoing maintenance hemodialysis were divided into the control group and the observation group according to the random numerical table,with 173 cases in each group.Patients in the control group was treated with levocarnitine injection alone,and patients in the observation group were treated with levocarnitine injection and fosinopril sodium tablets.Both groups were compared in terms of clinical effects,related laboratory indicators[serum creatinine(SCr),hemoglobin(Hb),C-reactive protein(CRP),albumin(Alb),tumor necrosis tumor factor-α(TNF-α)and interleukin-6(IL-6)],and the occurrence of adverse reactions.Results The total effective rate of treatment in the observation group was 98.27%,which was significantly higher than 91.91%in the control group,and the difference was statistically significant(P<0.05).The Hb(124.25±6.78)g/L and Alb(38.64±3.54)g/L of the observation group were higher than(112.36±7.45)and(34.32±2.67)g/L of the control group;the SCr(104.56±75.39)μmol/L,CRP(5.31±0.97)mg/L,TNF-α(10.16±1.07)pg/ml and IL-6(8.68±1.28)pg/ml of the observation group were lower than(130.53±76.68)μmol/L,(6.49±1.27)mg/L,(12.84±1.58)pg/ml,and(14.13±2.41)pg/ml of the control group;all the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 1.16%,which was lower than 6.36%in the control group,and the difference was statistically significant(P<0.05).Conclusion Levocarnitine injection in combination with fosinopril sodium tablets has significant clinical effect for maintenance hemodialysis,and it can effectively reduce the incidence of adverse reactions.
