摘 要:目的:探讨盆底四维超声在女性盆腔器官脱垂中的应用价值。方法:收集盆腔器官脱垂患者121例(患者组)及同期健康成年女性33例(对照组),患者组根据绝经状态分为A组(未绝经)和B组(绝经后)。3组均行盆底四维超声检查,测量膀胱颈移动度(BND)、膀胱尿道旋转角(URA)、静息期和Valsalva期膀胱尿道后角(PUVA)、肛提肌裂孔面积(LHA),计算静息期和Valsalva期肛提肌裂孔面积的差值(dLHA),比较各组间盆底超声参数差异。结果:3组的静息期LHA、Valsalva期LHA、dLHA、BND、URA及Valsalva期PUVA差异均有统计学意义(均P<0.05)。对照组与A、B组的静息期LHA、Valsalva期LHA、BND、URA比较,差异均有统计学意义(均P<0.05)。3组dLHA两两比较,差异均有统计学意义(均P<0.05)。A、B组的Valsalva期PUVA比较,差异有统计学意义(P<0.05)。结论:盆底四维超声可通过测量LHA、dLHA、BND、URA、PUVA等指标对盆腔器官脱垂进行诊断,对临床诊断具有很好的辅助作用,值得推广使用。Objective:To explore the application value of pelvic floor four-dimensional ultrasound in female pelvic organ prolapse.Methods:A total of 121 patients with pelvic organ prolapse(case group)and 33 healthy women(control group)were collected.Patients in the case group were divided into Group A and Group B according to menopausal status.The pelvic floor four-dimensional color Doppler ultrasound were used to measure bladder neck mobility(BND),bladder urethral rotation angle(URA),posterior urethrovesical angle(PUVA)and levator ani hiatus area(LHA)in resting stage and Valsalva stage,and the difference of LHA(dLHA)between resting stage and Valsalva stage,and the differences of pelvic floor ultrasound parameters among the groups were compared.Results:The LHA in resting stage,dLHA,BND,URA and LHA and PUVA in Valsalva stage had statistically significant differences in the three groups(all P<0.05).There were statistically significant differences between the control group and Group A and Group B of LHA in resting stage and Valsalva stage,BND,and URA(all P<0.05).The difference of dLHA among the three groups was statistically significant(P<0.05).The difference between Group A and Group B of PUVA in Valsalva stage was statistically significant(P<0.05).Conclusions:Pelvic floor four-dimensional ultrasound can diagnose pelvic organ prolapse by measuring LHA,dLHA,BND,URA,PUVA and other indicators.It has a better auxiliary effect on clinical diagnosis and is worthy of clinical promotion.
