第二产程中应用液状石蜡油对头位难产分娩结局的影响

摘　　要：目的探讨第二产程中应用液状石蜡油对头位难产分娩结局的影响。方法回顾性分析2020年5月至11月云南省怒江傈僳族自治州福贡县人民医院妇产科收治的72例头位难产患者的临床资料,根据第二产程助产干预方法不同分为常规组(36例)与石蜡组(36例)。常规组采用常规助产干预方法,石蜡组在常规干预的基础上加用液体石蜡油徒手旋转胎头。比较两组的分娩方式、第二产程及分娩并发症发生情况。结果两组的分娩方式比较,差异有统计学意义(P<0.05)。石蜡组的第二产程短于常规组,差异有统计学意义(P<0.05)。石蜡组的会阴损伤率、新生儿窒息率低于常规组,差异有统计学意义(P<0.05)。结论在头位难产患者第二产程应用液状石蜡油徒手旋转胎头能够显著缩短产程和降低剖宫产率,减轻产妇阴道受损程度,降低新生儿窒息率,改善母婴结局。Objective To investigate the influence of applying liquid paraffin oil in the second stage of labor on the outcome of headposition dystocia.Methods The clinical data of 72 cases of patients with cephalic dystocia admitted to People′s Hospital of Fugong County of Nujiang Lisu Autonomous Prefecture in Yunnan Province from May to November 2020 were analyzed retrospectively.According to the different intervention methods of the second stage of labor,they were divided into the routine group(36 cases)and the paraffin group(36 cases).The routine group was treated with conventional intervention,and the paraffin group was treated with liquid paraffin oil to rotate the fetal head by hand on the basis of conventional intervention.The mode of delivery,the second stage of labor and the incidence of delivery complications were compared between the two groups.Results The difference in delivery mode between the two groups was statistically significant(P<0.05).The second stage of labor in the paraffin group was shorter than that in the routine group,and the difference was statistically significant(P<0.05).The perineal injury rate and neonatal asphyxia rate in the paraffin group were lower than those in the conventional group,and the differences were statistically significant(P<0.05).Conclusion Using liquid paraffin oil to rotate the fetal head by hand in the second stage of dystocia at head position can significantly shorten the labor process and reduce the rate of cesarean section,reduce the degree of vaginal damage of puerpera,reduce the neonatal asphyxia rate,and improve the outcome of mother and baby.
