中老年慢性心力衰竭患者衰弱与赋权现状调查及相关性分析

摘　　要：目的探讨中老年慢性心力衰竭患者衰弱和赋权的现状,并分析两者之间的相关性。方法选取2020年3月至2021年2月广东省第二中医院心血管科住院的187例中老年慢性心力衰竭患者为调查对象。运用患者赋权量表和衰弱评估量表进行评估,通过Pearson相关性分析和多元线性回归分析探讨赋权对衰弱的影响关系。结果中老年慢性心力衰竭患者的赋权得分为(109.17±11.23)分,其中≤160分的患者为182例(97.3%),衰弱得分为(61.68±14.34)分,得分率为63.6%。不同年龄、有无合并并发症、心功能分级、生活自理能力患者的衰弱得分比较,差异有统计学意义(P<0.05)。赋权各维度得分与衰弱各维度得分呈负相关(P<0.05)。医患关系处理能力、社会支持、角色认识、医疗决策参与、病友支持是衰弱的独立影响因素(P<0.05)。结论中老年慢性心力衰竭患者的赋权能力相对较差,且衰弱情况相对严重,临床医护人员应注意提升患者的赋权水平,以减少衰弱带来的不良影响。Objective To explore the status of the empowerment and frailty in middle-aged and elderly patients with chronic heart failure,and analyze the correlation between them.Methods A total of 187 middle-aged and elderly patients with chronic heart failure in the Department of Cardiovascular,Guangdong Second Traditional Chinese Medicine Hospital from March 2020 to February 2021 were selected and evaluated by patient empowerment scale and infirmity assessment scale.Pearson correlation analysis and multiple linear regression analysis were used to explore the influence of empowerment on infirmity.Results The empowerment score of middle-aged and elderly patients with chronic heart failure was(109.17±11.23)points,including 182 patients with≤160 points(97.3%),and the frailness score was(61.68±14.34)points,with a scoring rate of 63.6%.There were statistically significant differences in fadiness scores of patients with different age,complication,heart function grade and self-care ability(P<0.05).The scores of empowerment dimensions were negatively correlated with those of weakness dimensions(P<0.05).Doctor-patient relationship management ability,social support,role recognition,medical decision-making participation and patient support were the independent factors of frailty(P<0.05).Conclusion The empowerment ability of the middle-aged and old patients with chronic heart failure were relatively poor,and the frailty situation were relatively serious.Clinical medical staff should pay attention to improving the patient′s empowerment level to reduce the adverse effects of frailty.
