摘  要:目的:开发一种新的重建算法,用于改善单光子发射型电子计算机断层扫描(SPECT)心肌门控的图像质量,为临床诊断提供依据。方法:利用SPECT不同相位图像之间的相关性,构造先验函数,推导迭代算法进行四维联合重建,基于临床数据和仿真数据对算法性能进行评估,并与传统三维重建方法得到的图像进行对比,验证其算法在提取心肌变化特征方面的优越性。结果:算法数值特性分析中,目标函数值随迭代次数变化曲线验证了算法在迭代时的单调收敛性;参数β和δ增大均会导致图像过平滑。四维联合重建与三维重建方法对比显示,四维联合重建可以有效地压低重建图像的噪声,改善重建图像的信噪比。三维重建+后滤波和四维重建的心肌活度曲线与临床结果呈显著相关(r=0.906,r=0.991;P<0.05);仿真数据的心肌活度不随时间变化,而3种不同重建得到的时间活度曲线归一化标准差分别为9.2%、6.4%和5.7%。基于四维重建算法得到的心肌活度随时间变化更贴近临床数据的参考值和仿真数据的真实值。结论:四维重建算法可以显著改善SPECT心肌门控成像质量,重建结果可以更好地反应心肌的形态变化和代谢情况,有利于临床上更好地对心肌功能进行评估和诊断。Objective:To develop a new reconstruction algorithm to improve the myocardial gated image quality of single-photon emission computed tomography(SPECT),and provide more basis of clinical diagnosis.Methods:The correlation among different phase images of SPECT was used to construct transcendental functions and deduce iterative algorithm for combined reconstruction with 4-dimension(4D).The performance of this algorithm was assessed on the basis of clinical data and simulation data,and was compared with the images from traditional threedimension(3D)reconstruction algorithm so as to verify the superiority of this algorithm on the aspect of extracting the change features of myocardium.Results:In numerical analysis for the features of algorithm,the curve that objectively functional value changed with the changing of iterations has verified the monotone convergence of algorithm in iterations,and the enlargements of and would lead to undue smooth of images.Compared with 3D reconstruction,the results of 4D reconstruction could effectively depress the noise of reconstructed images,and improve the signal to noise ratio(SNR)of reconstructed images.There were significant correlations between"3D reconstruction+post filter"and 4D reconstruction in activity-curve of myocardium with clinical results(r=0.906,r=0.991,P<0.05).The activity of myocardium of simulation data didn’t change with the changing of time,and the normalization standard deviation of time activity curve from three kinds of reconstructed images were 9.2%,6.4% and 5.7%,respectively.The changes of myocardial activity with the changing of time on the basis of 4D reconstruction algorithm were more close to the reference value of clinical data and the true value of simulation data.Conclusion:4D reconstructed algorithm can significantly improve the quality of SPECT myocardial gated imaging,and the reconstructed results can better reflect the morphologic change and metabolism of myocardium,which contributes to better assess and diagnose the myocardial function in clinica
