ABCLOVE嗓音训练法改善儿童声带小结发音功能的临床观察

摘　　要：目的探讨ABCLOVE嗓音训练法在儿童声带小结治疗中的应用,以及对患儿发声功能的临床改善效果。方法选取2019年1月至2021年3月收治的100例声带小结患儿为研究对象,利用随机数字表分组方式将患儿分为对照组和观察组各50例。对照组采用药物治疗及行为干预,观察组给予ABCLOVE嗓音训练法及行为干预治疗,每周1次,每次30分钟,共6周。比较两组干预前后电子喉镜下声带小结改善情况、声音嘶哑评估(grade roughness breathiness asthenia strained,GRBAS)和嗓音声学分析等指标。结果治疗后观察组总嘶哑度(grade,G)、粗糙声(roughness,R)评分较对照组下降明显(P<0.05),嗓音声学分析中基频微扰(jitter)、振幅微扰(shimmer)、声门噪声激发比(glottal to noise excitation ratio,GNE Ratio)指数观察组均显著低于对照组(P<0.05),最长发声时间(MPT)观察组较对照组明显延长(P<0.05),电子喉镜检查显示观察组治疗有效率、声带小结改善情况优于对照组(P<0.05),且两组治疗前后各参数均有显著改善(P<0.05)。结论ABCLOVE嗓音训练治疗儿童声带小结疗效显著,可有效改善患儿发音功能,降低嗓音嘶哑程度,改善患儿嗓音质量。Objective To investigate the application of ABCLOVE voice rehabilitation therapy in the treatment of children with vocal nodules and its clinical improvement effect on vocal function.Methods A total of 100 children with vocal nodules admitted from January,2019 to March,2021 were selected and were divided into two groups,with 50 cases in each group,as control group and observation group by random number table grouping.The control group received drug therapy and behavioral intervention and the observation group received ABCLOVE voice rehabilitation therapy and behavioral intervention,once a week,30 minutes each time for 6 weeks.The improvement of vocal nodules and voice quality were evaluated and compared by electronic laryngoscopy,assessment of hoarseness(GRBAS)and acoustic analysis of voice between the two groups before and after intervention.Results Postoperatively the total hoarseness and roughness score of the observation group decreased significantly compared with the control group(P<0.05),and the jitter,shimmer and GNE Ratio of acoustic analysis of voice in the observation group were lower than those in the control group(P<0.05).The maximum vocalation time(MPT)in the observation group was significantly longer than the control group(P<0.05),the effectiveness of observation group was significantly higher than the control group by the electronic laryngoscopy,and the improvement of vocal nodules was better than the control group(P<0.05).After treatment,the both two groups were improved compared with those before treatment and the differences were statistically(P<0.05).Conclusion ABCLOVE voice rehabilitation therapy has significant efficacy in the treatment of children with vocal nodules,which can effectively improve the voice function and quality,reduce the degree of hoarseness with high safety,and be worth to be popularized and utilized.
