“提壶揭盖”中西融通 用中医思维方式解决临床问题——附3例胸腔积液病例报告

摘　　要：在临床实践中,医生常会遇到运用片面的医学知识难以解释或处理的临床现象或问题,由此使我们联想到“中西医结合”方法,即采用中医学和西医学理论和方法各取其长、互补其短,从而解决实际问题。本文介绍3例胸腔积液和急性肾功能不全患者,因采用常规保守治疗方法(给予足量利尿剂)效果不佳,遂间断给予胸膜腔穿刺抽取胸水,引流胸水的同时可见尿量增加,水肿消退,呼吸困难等症状亦得到缓解。该文分别从中医和西医角度分析并阐述其机制,在寻找理论契合点的同时,提供中西医结合治疗的思路。In clinical practice,doctors often encounter clinical phenomena and problems that are difficult to explain and manage with one-sided clinical medical knowledge.Therefore,we often think of the method of"integration of Chinese and Western medicine",which means that the theories and methods of Chinese medicine and Western medicine are complementary to each other or in other words,"to learn from others'strong points to offset one's weaknesses"to solve practical problems.This article introduces a patient with pleural effusion and acute renal insufficiency.The effect of routine conservative treatment using sufficient diuretics for the patient was poor,so pleural puncture of western method was used intermittently to extract pleural fluid.It could be seen that the urine volume increased and the symptoms of dyspnea were relieved.Then we analyzed the mechanism of this phenomenon from the perspective of Chinese and Western medicine,in order to find the theoretical similarities between them,and in the mean time,provide ideas of using integration of Chinese and Western medicine for treatment.
