摘  要:目的观察前列通窍胶囊(QTC)对膀胱出口部分梗阻(pBOO)大鼠膀胱组织病理及缺氧诱导因子-1α(HIF-1α)蛋白表达的影响。方法28只雄性SD大鼠中随机选出7只纳入假手术组,对余下21只均施以pBOO造模手术后随机分为pBOO模型组、QTC低剂量组和QTC高剂量组,每组各7只。术后第1天起对各组大鼠相应予以QTC粉末混悬液或蒸馏水灌胃,4周后通过过量麻醉处死所有大鼠,记录体重、膀胱湿重并计算膀胱指数;HE染色法检测膀胱组织病理;免疫组织化学和免疫印迹试验检测膀胱组织中HIF-1α蛋白表达情况。结果与假手术组相比,pBOO模型组大鼠出现明显的膀胱代偿性肥大、平滑肌间隙减小及纤维化,膀胱湿重、膀胱指数及逼尿肌层厚度均显著增加(P均<0.0001),同时,膀胱组织中HIF-1α蛋白表达显著升高(P<0.01);QTC低剂量组大鼠膀胱湿重、膀胱指数、逼尿肌层厚度及膀胱组织中HIF-1α蛋白表达与pBOO组无统计学差异(P均>0.05),亦伴随明显的膀胱病理组织学改变;而QTC高剂量组大鼠膀胱湿重、膀胱指数及逼尿肌层厚度均低于pBOO组(P均<0.05),膀胱组织中HIF-1α蛋白表达明显下调(P<0.01),且膀胱组织病理改变减轻。结论高剂量QTC可抑制pBOO所导致的膀胱湿重和膀胱指数升高,改善代偿性肥大、逼尿肌层厚度增加、平滑肌间隙减小及纤维化等膀胱组织病理改变,并下调膀胱组织中HIF-1α蛋白表达,上述是QTC减轻pBOO所致膀胱损伤的部分作用机理。Objective To study the effects of Qianlie Tongqiao Capsule(QTC)on the pathological changes and HIF-1αexpression in the bladder of rats with partial bladder outlet obstruction(pBOO).Methods Seven of 28 male Sprague Dawley rats were randomly enrolled into the sham surgery group(Sham group),and then the other 21 rats were enrolled into pBOO model group,low-dosage QTC group(QTC-L group),and High-dosage QTC group(QTC-H group)after suffering pBOO-establishment surgery,7 in each group.Rats of all the groups were accordingly given with QTC powder suspension or distilled water via intragastric administration from the first day after surgery.Four weeks later,all rats were executed by excessive anesthesia,then the body weight(BW),bladder wet weight(BWW)were recorded and the bladder index was calculated accordingly.Pathological changes of bladder tissues were detected by hematoxylin and eosin staining,and their HIF-1αexpression were analyzed by immunohistochemistry and western blot.Results Remarkable compensatory hypertrophy,coarctation of smooth muscle bundle and fibrosis were observed in the bladder tissue of pBOO rat models as compared with the controls,and their BWW,BI and detrusor wall thickness were all significantly increased(P all<0.0001);Meanwhile,HIF-1αexpression in bladder tissues of pBOO rats obviously elevated(P<0.01).There were no statistical differences in these observational indexes mentioned above between the QTC-L group and pBOO group(P all>0.05).However,BWW,BI,and detrusor wall thickness of rats in QTC-H group were decreased as compared with these in pBOO group(P all<0.05),HIF-1αexpression in bladder tissues was down-regulated(P<0.01)and pathological changes in bladder tissue were obviously ameliorated.Conclusion High-dosage QTC can inhibit the increase of BWW and BI induced by pBOO,simultaneously mitigate pathological changes such as compensatory hypertrophy,detrusor wall thickening,coarctation of smooth muscle bundle and fibrosis,down-regulate the expression of HIF-1αin the bladder tissue,which may
