摘　　要：目的观察丘墟透照海针刺治疗对缺血性脑卒中后足内翻患者即刻积分肌电图(iEMG)的影响。方法缺血性脑卒中后足内翻患者26例行丘墟透照海针刺治疗,采用表面肌电图记录治疗前后足内翻患者患/健侧胫前肌和腓肠肌行被动牵拉-放松和主动踝背屈-放松即刻iEMG值的变化。结果被动牵拉状态、被动牵拉后放松状态、主动踝背屈状态和踝背屈后放松状态,患侧胫前肌iEMG值均低于健侧胫前肌(P<0.05)。主动踝背屈状态下,针刺治疗即刻患侧胫前肌iEMG值高于针刺治疗前[23.980(10.158,69.845)μV·s vs.24.245(6.720,54.283)μV·s](P<0.05)。主动踝背屈放松状态下,针刺治疗即刻患侧胫前肌iEMG值与治疗前相仿,且针刺治疗即刻患侧腓肠肌主动踝背屈状态和主动踝背屈放松状态iEMG值与治疗前均相仿(P>0.05)。结论丘墟透照海针刺治疗能增加患侧主动踝背屈状态下的即刻iEMG值,有效提高脑卒中后足内翻患者胫前肌单位时间收缩能力。Objective To observe the effect of Qiuxu through Zhaohai(QtZ)acupuncture therapy on the immediate integration electromyogram(iEMG)value in the patients with strephenopodia after ischemic stroke.Methods The QtZ acupuncture therapy was performed in 26 patients with strephenopodia after ischemic stroke.The immediate iEMG values of the anterior tibialis muscle and the gastrocnemius muscle on the diseased and healthy sides were recorded with surface electromyography,which were performed in the status of passive traction/relaxation and active ankle dorsiflexion/relaxation before and after treatment.Results In the status of passive traction,passive post-traction relaxation,active dorsiflexion or post-ankle dorsiflexion relaxation,the iEMG values of the diseased tibialis anterior muscle were lower than those of healthy one(P<0.05).In the status of active dorsiflexion of ankle,the iEMG value of diseased tibialis anterior muscle immediately after acupuncture therapy was higher than that before[23.980(10.158,69.845)μV·s vs.24.245(6.720,54.283)μV·s](P<0.05).In the active ankle dorsiflexion relaxation status,the iEMG value of diseased tibialis anterior muscle immediately after acupuncture therapy was similar to that before(P>0.05).So did the iEMG values of diseased gastrocnemius muscle in active ankle dorsiflexion and active ankle dorsiflexion relaxation status immediately after acupuncture therapy(P>0.05).Conclusion The QtZ acupuncture therapy can immediately increase the immediate iEMG value on the diseased side in the status of active dorsiflexion of ankle and effectively improve the contraction ability of the tibialis anterior muscle in the patients with strephenopodia after ischemic stroke.
