摘  要:目的探讨畸形精子症患者中氧化应激相关指标与精子DNA损伤变化的相关性,以期为男性不育诊治提供理论依据。方法选取2019年1月至2019年8月在河北医科大学第四医院生殖医学科诊治的101例男性不育症患者作为研究对象,按精子形态将患者精液标本分为两组,A组为精子正常形态≥4,B组为精子正常形态<4,即畸形精子症组,按世界卫生组织(WHO)精液分析第5版要求进行精液常规分析,使用伊红-苯胺黑染色方法进行精子膜完整性检测,吖啶橙荧光染色方法检测精子DNA损伤,氧化应激试剂盒检测精浆丙二醛(MDA)水平、总抗氧化能力(TAC)、超氧化物歧化酶(SOD)活性。结果与A组比较,B组的前向运动精子降低,组间比较差异有统计学意义(P<0.05);两组的精液量、精子浓度比较,差异无统计学意义(P>0.05,表1)。与A组比较,B组的精子膜完整性降低,精子DNA损伤增加,差异均有统计学意义(均P<0.05)。与A组比较,B组的TAC和SOD水平明显降低,MDA水平显著增高,差异均有统计学意义(均P<0.05)。精子正常形态率与精子DNA损伤(r=-0.492)、活性氧水平(r=-0.645)和MDA(r=-0.439)均呈负相关(均P<0.05)。精子正常形态率与TAC(r=0.623)、SOD(r=0.547)水平均呈正相关(均P<0.05)。结论氧化应激水平的改变可导致精子活力降低、精子膜完整性降低和精子DNA损伤增加。对氧化应激的检测和抗氧化的治疗可能会预防氧化应激引起的精子形态异常以及相关指标的改变,对于提高男性生育力有积极作用。Objective To investigate the changes and correlation of oxidative stress related indexes and sperm DNA damage in teratozoospermia patients,in order to provide theoretical basis for the diagnosis and treatment of male infertility.Methods A total of 101 male semen samples from the department of reproductive medicine in our hospital were selected and divided into two groups according to sperm morphology.Group A was normal sperm morphology≥4,and group B was normal sperm morphology<4,teratoospermia group.Routine semen analysis was performed according to the requirements of WHO semen analysis edition 5.sperm membrane integrity was detected by eosin-aniline black staining method,sperm DNA damage was detected by acridine orange fluorescence staining method,sperm malondialdehyde(MDA)level,total antioxidant capacity(TAC)and superoxide dismutase(SOD)activity were detected by oxidative stress kit.Results①The semen volume of group A and group B were(2.84±1.61)mL and(2.79±1.13)mL,and the sperm concentration was(44.55±17.93)×106/mL and(39.41±15.75)×106/mL,respectively,with no significant difference(P>0.05).The percentage of forward sperm in group A was(47.47±9.34)%,which was significantly higher than that in group B(42.18±12.74)%(P<0.05).②The sperm membrane integrity of group A was(82.27±9.02)%,which was higher than that of group B(76.56±11.53)%,and the difference was statistically significant(P<0.05).The percentage of sperm DNA damage in group A was decreased by(12.90±2.39)%,compared with(17.88±3.53)%in group B,the difference was statistically significant(P<0.05).③The serum MDA level in group A was(8.13±1.69)nmol/mL,which was lower than that in group B(11.24±2.50)nmol/mL,and the difference was statistically significant(P<0.05).The levels of TAC and SOD in sperm plasma of group A were increased compared with group B,and the differences were statistically significant(P<0.05).④Sperm DNA damage(r=-0.492),reactive oxygen species(r=-0.645)and MDA(r=-0.439)were negatively correlated with sperm morphology(P<0.0
