摘　　要：目的探讨基质金属蛋白酶-7(MMP-7)、基质金属蛋白酶-28(MMP-28)在特发性肺纤维化(IPF)诊断及严重程度评估中的临床价值。方法选2018年9月至2019年12月在宁夏回族自治区人民医院就诊的30例IPF患者为实验组,30例类风湿性关节炎相关间质性肺病(RA-ILD)及30例健康体检者为对照组。测定所有受试者血浆中MMP-7和MMP-28的水平,分析其与FVC%、DLCO%及HRCT评分的相关性及意义。结果1.(1)IPF组患者血浆MMP-7水平与RA-ILD组相比差异无统计学意义(P>0.05),较健康对照组明显升高,差异有统计学意义(P<0.05);(2)IPF组患者血浆MMP-28水平明显高于RA-ILD组以及健康对照组,差异均有统计学意义(P<0.05)。2.IPF组血浆中MMP-7、MMP-28水平分别与FVC%、DLCO%呈负相关;与HRCT评分呈正相关。3.经ROC曲线分析结果显示,MMP-7诊断IPF的曲线下面积为0.97,MMP-28的曲线下面积为0.98,MMP-7、MMP-28联合曲线下面积为0.99;MMP-28鉴别IPF与RA-ILD的曲线下面积为0.71。结论MMP-7、MMP-28水平可作为IPF诊断及病情严重程度评估的生物标记物;而MMP-28或可用于IPF的鉴别诊断。Objective To investigate the clinical value of matrix metalloproteinase-7(MMP-7)and matrix metalloproteinase-28(MMP-28)in the diagnosis and severity assessment of idiopathic pulmonary fibrosis(IPF).Methods It selected 30 patients with IPF from Ningxia people′s hospital as the study group,30 patients with rheumatoid arthritis associated with interstitial lung disease as the RA-ILD group and 30 healthy people as the control group.The serum concentration of MMP-7 and MMP-28 was measured,and the correlation of MMP-7 and MMP-28 with pulmonary function indicators was analyzed.Results 1.The MMP-7 concentration showed no significant difference in the IPF group and the RA-ILD group(P>0.05),but it was much higher than that in the healthy control group(P<0.05).The level of MMP-28 increased more significantly in the IPF group than in the RA-ILD group and the healthy control group.3.The levels of MMP-7 and MMP-28 showed negative correlation with FVC%and DLCO%,and showed positive correlation with HRCT score in the IPF group.4.The ROC curve was drawn to evaluate the diagnostic value of MMP-7 and MMP-28 in the IPF group.The area under curve(AUC)of MMP-7 was 0.97,the AUC of MMP-28 was 0.98,and the AUC of combination of MM-7 and MMP-28 was 0.99.The AUC of serum concentration of MMP-28 differentiating IPF from RA-ILD was 0.71.Conclusion MMP-7 and MMP-28 can be used as biomarkers for the diagnosis and severity assessment of IPF.MMP-28 can distinguish IPF from other ILDs.
