摘 要:目的探讨长期美沙酮维持治疗(MMT)对海洛因成瘾者大脑突显网络(SN)对默认网络(DMN)与执行控制网络(ECN)连接的影响。方法招募26例短期戒断的海洛因成瘾者(SA组)、25例长期MMT者(MMT组)和43例健康对照者(HC组)为研究对象,进行静息态功能磁共振成像(rs-fMRI)数据采集。采用独立成分分析方法提取DMN、ECN和SN。比较3组间SN-DMN,SN-双侧ECN的功能连接强度,并分析与美沙酮治疗时间的相关性。结果3组间SN-DMN、SN-双侧ECN的功能连接方差分析均存在显著性差异(F=4.12,P=0.02;F=9.05,P=0.00;F=8.02,P=0.00)。事后分析,与HC组相比,SA组SN-左侧ECN功能连接显著降低(P=0.02,LSD校正),MMT组SN-左侧ECN、SN-右侧ECN功能连接显著增强(P=0.00,LSD校正;P=0.01,LSD校正)。与SA组相比,MMT组SN-DMN、SN-左侧ECN、SN-右侧ECN功能连接显著增强(P=0.005,LSD校正;P=0.00,LSD校正;P=0.00,LSD校正)。网络间功能连接与美沙酮治疗时间无相关性(P>0.05)。结论MMT有利于海洛因成瘾者大脑SN与DMN和双侧ECN的功能连接,促进脑功能的恢复。Objective To investigate the effect of long-term methadone maintenance treatment(MMT)on the cerebral functional connectivity of salience network(SN),default mode network(DMN)and executive control network(ECN)in patient with heroin addicts.Methods Twenty-six patients with short-term abstinent heroin addicts(SA group),25 patients with long-term MMT(MMT group)and 43 healthy controls(HC group)were recruited and resting-state functional magnetic resonance imaging(rs-fMRI)data were further collected.The DMN,ECN and SN were extracted and obtained by using independent component analysis.The SN-DMN and SN-bilateral ECN were compared to measure the functional connectivity among the three groups.The correlation between the functional connectivity and duration of methadone treatment were further analyzed.Results There were significant differences in functional connectivity of SN-DMN and SN-bilateral ECN among these three groups(F=4.12,P=0.02;F=9.05,P=0.00;F=8.02,P=0.00).According to the post-hoc analysis,compared with HC group,the functional connectivity of SN-left ECN in SA group was significantly decreased(P=0.02,LSD correction),and that of SN-left ECN and SN-right ECN in MMT group were significantly increased(P=0.00,LSD correction;P=0.01,LSD correction).Compared with SA group,the functional connectivity of SN-DMN,SN-left ECN and SN-right ECN were significantly enhanced in MMT group(P=0.005,LSD correction;P=0.00,LSD correction;P=0.00,LSD correction).There was no significant correlation between functional connectivity and duration of methadone treatment(P>0.05).Conclusion MMT is beneficial to the formation of functional connection of SN-DMN and SN-bilateral ECN in patient with heroin addicts,promoting the recovery of cerebral function.
