摘 要:目的评估正常肾脏3D酰胺质子转移(APT)成像的可行性。方法应用3.0T MR扫描仪对34例健康志愿者进行肾脏3D APT成像,其中18例为双肾同时扫描,16例为单肾分别扫描。利用飞利浦后处理工作站(ISP)融合T2WI图像和APT图像。由2位观察者分别对肾脏APT图像进行主观评分,并在融合图像肾门中心层面及上下层面的双侧肾脏皮质和髓质各放置1个感兴趣区(ROI),测量数据分别取平均值。2位观察者分别于T2WI和APT图像肾门最大截面,选择肾脏最厚处测量肾脏厚度。采用SPSS软件的组内相关系数(ICC)进行一致性检验。利用ICC比较T2WI和APT图像间肾脏厚度的一致性,以评价APT图像变形程度。利用Wilcoxon检验比较双侧肾脏主观评分的差异、双肾自身皮髓质APT值的差异和双肾间皮质、髓质APT值的差异。结果2位观察者对肾脏APT图像的主观评分一致性良好(ICC>0.75)。2位观察者测得双肾同时扫描及单肾分别扫描的肾脏APT值的一致性良好(ICC>0.75)。双肾同时扫描,右肾皮质、右肾髓质、左肾皮质和左肾髓质的APT值分别为(1.79±1.37)%、(0.84±0.64)%、(1.69±0.97)%和(2.10±1.17)%。单肾分别扫描,右肾皮质、右肾髓质、左肾皮质和左肾髓质的APT值分别为(1.52±0.28)%、(0.91±0.26)%、(1.46±0.35)%和(1.91±0.37)%。双肾同时扫描与单肾分别扫描趋势相同。右侧肾脏皮质和髓质间、左侧肾脏皮质和髓质间APT值差异均有统计学意义(P<0.05)。肾脏T2WI和APT图像间肾脏厚度一致性较好(ICC均>0.8)。结论正常肾脏不同区域的APT成像可重复定量测量,双侧肾脏间髓质APT值差异较大,需后续校准试验。Objective To evaluate the feasibility of 3D amide proton transfer(APT)imaging in healthy kidney.Methods Thirty-four healthy young volunteers were recruited and underwent 3D APT imaging of kidney on a 3.0T MR scanner,among them,18 volunteers scanned both kidneys at the same time,and 16 volunteers scanned one kidney at a time.The fusion images of T2WI and APT images were generated on intellispace portal(ISP).The subjective image scores of APT images were graded by two observers respectively.In the fusion image,a region of interest(ROI)was placed on the central level of the renal hilum and on the bilateral renal cortex and medulla at the upper and lower levels,and the measured data were averaged.The thickest part of the kidney was selected by two observers to measure the thickness of the kidney at the maximum section of renal hilum in T2WI and APT images,respectively.The intraclass correlation coefficient(ICC)of SPSS software was used for consistency test.The ICC was used to compare the consistency of kidney thickness between T2WI and APT images,in order to evaluate the distortion of APT images.Wilcoxon test was used to compare the difference of bilateral kidney subjective scores,the differences of APT values between the cortex and medulla of each kidneys,differences of APT values of cortex and medulla between two kidneys.Results The consistency of subjective evaluation of the APT image quality by two observers was good(ICC>0.75).The consistency of the APT values of both kidneys scanned simultaneously and separately that measured by two observers was good(ICC>0.75).When both kidneys were scanned at the same time,the APT values of right renal cortex,right renal medulla,left renal cortex,and left renal medulla were(1.79±1.37)%,(0.84±0.64)%,(1.69±0.97)%and(2.10±1.17)%,respectively.When each kidney was scanned separately,the APT values of right renal cortex,right renal medulla,left renal cortex and left renal medulla were(1.52±0.28)%,(0.91±0.26)%,(1.46±0.35)%and(1.91±0.37)%,respectively.Which illustrated that th
