摘 要:目的探讨胰腺全器官灌注成像的技术方法及灌注参数测量的稳定性。方法对30例正常胰腺用640层CT进行灌注扫描,并用其自带软件进行图像的后处理,由2名经验丰富的医师对动脉血流量(AF)值、等效血容量(Equiv BV)值及血流萃取(FE)值进行测量,其中1名高年资医师1周后重复测量,采用组内相关系数(ICC)比较2名医师间及高年资医师重复测量结果间的一致性;同时计算扫描全过程产生的X线剂量,并使用t检验比较灌注扫描全过程和同机型上腹部常规检查(平扫+3期强化)所产生的辐射剂量有无差别。结果所有研究对象均成功地完成了23个数据包的全器官灌注扫描,AF灌注图的获取利用最大斜率法(MS),Equiv BV灌注图及FE灌注图的获取利用Patlak法;不同医师各参数测量结果的ICC为0.795~0.877,同一医师重复测量间的ICC为0.776~0.903,均有很好的一致性;扫描全过程产生的X线平均剂量为(10.99±0.04)mSv,和常规上腹部CT检查产生的剂量比较,无统计学差异(P>0.05)。结论640层全胰腺单血供多参数灌注成像检查过程及后处理简单,灌注参数测量的稳定性好,辐射剂量低,值得临床推广应用。Objective To study the technical methods of pancreatic whole organ perfusion imaging and the stability of perfusion parameters measurement.Methods Thirty cases of normal pancreas were scanned with 640-slice CT,and the images were post-processed with its own software.Two experienced physicians measured the arterial blood flow(AF),equivalent blood volume(Equiv BV)and blood flow extraction(FE).One of the senior doctors repeated the measurement one week later,and the intraclass correlation coefficients(ICC)were used to compare the consistency between the two doctors and the repeated measurement results of the senior doctor.At the same time,the X-ray dose produced in the whole process of scanning was calculated,and i-test was conducted to compare the difference of radiation dose between the whole process of perfusion scan and routine examination of upper abdomen(plain scan+three-phase enhancement)of the same model.Results All subjects successfully completed 23 data packets of whole organ perfusion scan.AF perfusion map was obtained using maximum slope method(MS),Equiv BV and FE perfusion map was obtained using the Patlak method.The ICC of the measurement results of each parameter of different physicians was 0.795-0.877,and the ICC between repeated measurements by the same physician was 0.776-0.903,all of which were highly consistent.The average X-ray dose produced during the whole scanning process was(10.99±0.04)mSv,which showed no significant difference with the average dose of X-ray produced during the whole scanning process and that produced by routine upper abdomen CT examination(P>0.05).Conclusion Perfusion imaging of 640-slice whole pancreas with single blood supply and multi-parameter has the advantages of simple processing and post-processing,good stability of perfusion parameters and low radiation dose,so it is of good clinical significance.
