摘　　要：[目的]探究c-kit蛋白、Ki67、CyclinD1、CyclinD3在上消化道小间质瘤(sGIST)中的表达水平及其病理意义。[方法]入选80例经内镜切除并病理证实的上消化道sGIST(直径≤2 cm)患者设为sGIST组,外科手术切除20例直径在>2~5 cm胃间质瘤设为间质瘤组、同期病理证实为恶性间质瘤(直径>5~8 cm)的20例患者设为恶性组,均应用免疫组织化学方法测定c-kit蛋白、Ki67、CyclinD1、CyclinD3表达情况。[结果]恶性组患者的c-kit蛋白、Ki67、CyclinD1、CyclinD3阳性表达水平显著大于间质瘤组及sGIST组(P<0.05)。不同NIH危险度分级的患者,其c-kit蛋白、Ki67、CyclinD1、CyclinD3表达的差异有统计学意义(P<0.05),且危险度与c-kit蛋白、Ki67、CyclinD1、CyclinD3表达水平呈正相关(P<0.05)。[结论]c-kit蛋白、Ki67、CyclinD1、CyclinD3表达在上消化道sGIST已经出现表达,而且其表达程度与间质瘤NIH危险分级密切相关,可作为临床判断上消化道sGIST恶性程度的指标,对于上消化道sGIST预后判断具有较大临床参考意义。[Objective]To investigate the expression levels of c-kit protein,Ki67,CyclinD1 and cyclind3 in gastrointestinal stromal tumors(GIST)and their pathological significance.[Methods]From December 2017 to May 2020,80 patients with small stromal tumor(diameter≤2 cm)in upper gastrointestinal tract in Ningde municipal hospital of Ningde normal university were selected as small stromal tumor group,20 patients with gastric stromal tumor(diameter from 2 to 5 cm)were selected as gastric stromal tumor group,and 20 patients with malignant stromal tumor confirmed by pathology were selected as malignant group.The expression of c-kit protein,Ki67,CyclinD1 and cyclinD3 was detected by immunohistochemistry.The expression levels of each protein molecule were compared among different pathological mitotic figures and NIH risk grades.[Results]The positive expression levels of c-kit protein,Ki67,CyclinD1 and cyclind3 in malignant group were significantly higher than those in gastric stromal tumor group and small stromal tumor group(P<0.05).In small gastrointestinal stromal tumors,patients with different NIH risk grades,had statistically significant differences in the expression of c-kit protein,Ki67,CyclinD1,and CyclinD3(P<0.05).Spearman analysis showed that the positive expression levels of c-kit protein,Ki67,CyclinD1 and cyclind3 were positively correlated with each other(P<0.05).[Conclusion]The expressions of c-kit protein,Ki67,CyclinD1,and CyclinD3 are closely related to the mitosis of gastrointestinal stromal tumors and NIH risk grade.They can be used as indicators for clinical judgment of the malignant degree of small gastrointestinal stromal tumors,and have some clinical references for their prognostic judgment.
