抗凝抗脂联合中医康复对预防高空坠落伤术后长期卧床患者下肢深静脉血栓形成的影响

摘　　要：目的:观察抗凝抗脂联合中医康复对高空坠落伤后长期卧床患者下肢深静脉血栓(DVT)形成的预防效果。方法:将60例接受手术治疗的高空坠落伤患者作为研究对象,随机分为研究组和对照组各30例。对照组给予氯吡格雷片、阿托伐他汀钙片联合常规护理进行预防治疗,研究组在对照组的基础上联合中医康复治疗。14 d为1个疗程,共治疗4个疗程。观察比较2组深静脉血栓的形成情况、血液流变学指标及炎症因子[高敏C-反应蛋白(hs-CRP)、白细胞介素-8(IL-8)]水平变化。结果:治疗3、4个疗程后,研究组深静脉血栓形成率分别为10.00%、20.00%,对照组分别为23.33%、43.33%,2组比较,差异均有统计学意义(P<0.05)。治疗后,研究组红细胞压积、全血黏度、纤维蛋白原水平均低于对照组,差异有统计学意义(P<0.05)。治疗前,2组hs-CRP、IL-8水平比较,差异无统计学意义(P>0.05);治疗后,2组hs-CRP、IL-8水平均较治疗前降低(P<0.05),且研究组2项指标均低于对照组(P<0.05)。结论:对高空坠落伤术后长期卧床患者给予抗凝抗脂联合中医康复治疗,可有效预防DVT的形成,改善患者血液流变学指标,减轻机体炎症反应。Objective:To observe the preventive effect of anticoagulation and anti-fat combined with Chinese medicine rehabilitation on deep venous thrombosis(DVT)in long-term bedridden patients after operation for injuries due to high falls.Methods:A total of 60 cases of patients receiving operation for injuries due to high falls were selected as the study subjects and randomly divided into the study group and the control group,with 30 cases in each group.The control group was treated with clopidogrel tablets and atorvastatin tablets combined with routine nursing for preventive treatment,and the study group was additionally treated with Chinese medicine rehabilitation based on the treatment of the control group.Both groups were treated for four courses,with 14 days being one course.The changes in deep vein thrombosis,hemorheology indexes and inflammatory factors levels including high-sensitivity C-reactive protein(hs-CRP)and interleukin-8(IL-8)in the two groups were observed and compared.Results:After three and four courses of treatment,the formation rates of deep vein thrombosis were 10.00%and 20.00%in the study group and 23.33%and 43.33%in the control group,differences being significant(P<0.05).After treatment,hematocrit,whole blood viscosity and fibrinogen level in the study group were lower than those in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of levels of hs-CRP and IL-8 between the two groups(P>0.05).After treatment,the levels of hs-CRP and IL-8 in the two groups were decreased when compared with those before treatment(P<0.05),and the two levels in the study group were lower than those in the control group(P<0.05).Conclusion:The therapy of anticoagulation and anti-fat combined with Chinese medicine rehabilitation for long-term bedridden patients after operation for injuries due to high falls can effectively prevent DVT formation,improve hemorheology indexes and relieve inflammatory responses in the body.
