三氧自体血疗法联合药物治疗老年带状疱疹后神经痛的疗效分析

摘　　要：目的研究三氧自体血疗法(ozone autohemotherapy,O3-AHT)联合口服药物治疗老年带状疱疹后神经痛(postherpetic neuralgia,PHN)的疗效和安全性。方法回顾分析2018年1月—2020年12月期间60例老年PHN患者,按治疗方法不同分为两组:A组(n=30)给予口服药物治疗2周,B组(n=30)给予O3-AHT联合口服药物治疗2周。采用视觉模拟评分(visual analogue scale,VAS)对所有PHN患者治疗前和治疗后1周、4周、12周的疼痛程度进行评估,采用比兹堡睡眠质量指数(pittsburge sleep quality index,PSQI)对所有PHN患者治疗前和治疗后4周、12周的睡眠质量进行评估。结果A组治疗前的VAS评分为(5.37±0.79)分,治疗后第1周、第4周以及第12周的VAS评分分别为(3.48±0.85)分、(3.11±0.85)分、(2.93±0.83)分。B组治疗前的VAS评分为(5.48±0.87)分,治疗后第1周、第4周以及第12周的VAS评分分别为(2.56±0.77)分、(2.32±0.69)分、(2.20±1.19)分。A组治疗前PSQI评分为(16.35±1.76)分,治疗后第4周、第12周PSQI评分为(11.65±1.46)分、(10.35±1.36)分。B组治疗前PSQI评分为(16.30±1.69)分,治疗后第4周、12周PSQI评分为(8.46±1.27)分、(7.26±1.29)分。两组治疗后随访第1周、第4周的VAS评分均较同组内治疗前和前一个时间点显著降低,差异有统计学意义(t=-19.383、-3.058、-22.801、-2.295,P<0.05)。两组治疗后随访12周和4周的VAS评分的组内比较,差异无统计学意义(t=-1.991、-0.721,P>0.05)。两组间治疗后随访的比较,B组治疗后同一时间点的VAS评分均较A组低,差异有统计学意义(t=4.093、3.673、2.534,P<0.05)。两组治疗后随访第4周和第12周的PSQI均较治疗前显著降低,差异有统计学意义(P<0.05),两组间治疗后随访的比较,B组治疗后同一时间点的PSQI评分均较A组低,差异有统计学意义(P<0.05)。结论O3-AHT联合口服药物治疗可显著改善老年PHN患者的疼痛程度和睡眠质量,优于单纯药物治疗。Objective To study the efficacy and safety of ozone autohemotherapy(O3-AHT)combined with oral drugs in the treatment of postherpetic neuralgia(PHN)in the elderly.Methods A retrospective analysis of 60 elderly PHN patients from January 2018 to December 2020 was divided into two groups according to different treatment methods:group A(n=30)was given oral medication for 2 weeks,group B(n=30)was given O3-AHT combined with oral medication for 2 weeks.The visual analogue scale(VAS)was used to evaluate the pain level of all PHN patients before treatment and 1 week,4 weeks,and 12 weeks after treatment.The Pittsburge sleep quality index(PSQI)was used to assess the sleep quality of all PHN patients before treatment and 4 weeks and 12 weeks after treatment.Results The VAS score of group A before treatment was(5.37±0.79)points,and the VAS scores of the first week,fourth week,and 12th week after treatment were(3.48±0.85)points,(3.11±0.85)points,and(2.93±0.83)points.The VAS scores of group B before treatment were(5.48±0.87)points,and the VAS scores of the first week,the fourth week,and the 12th week after treatment were(2.56±0.77)points,(2.32±0.69)points,and(2.20±1.19)points.The PSQI scores of group A before treatment were(16.35±1.76)points,and the PSQI scores were(11.65±1.46)points and(10.35±1.36)points at the 4th and 12th weeks after treatment.The PSQI score of group B before treatment was(16.30±1.69)points,and the PSQI scores of the 4th and 12th weeks after treatment were(8.46±1.27)points and(7.26±1.29)points.The VAS scores of the two groups in the first and fourth weeks of follow-up after treatment were significantly lower than those in the same group before and at the previous time point,and the difference was statistically significant(t=-19.383,-3.058,-22.801,-2.295,P<0.05).There was no statistically significant difference in the VAS scores between the two groups after 12 weeks and 4 weeks of follow-up after treatment(t=-1.991,-0.721,P>0.05).Compared the follow-up after treatment between the two groups,the V
