摘  要:目的探讨神经肌肉电刺激联合早期康复训练对前交叉韧带重建术后腘绳肌功能的影响。方法方便选取2018年1月—2020年12月福建医科大学附属第一医院康复医学科收治的70例前交叉韧带断裂患者为研究对象,全部患者均取自体腘绳肌进行前交叉韧带重建,随机分为两组。对照组35例,术后予以患者早期腘绳肌康复训练;研究组35例,在对照组基础上加用神经肌肉电刺激。干预后,测定两组双侧腘绳肌等长肌力与电机械延迟,以IKDC量表评价两组膝关节功能。结果研究组干预后患侧腘绳肌等长肌力(31.95±4.82)N·m/kg、IKDC评分(61.77±4.38)分,均高于对照组,差异有统计学意义(t=4.280、6.802,P<0.05)。研究组干预后患侧半腱肌电机械延迟(27.45±2.23)ms、股二头肌电机械延迟(29.74±3.06)ms,均低于对照组,差异有统计学意义(t=5.292、8.916,P<0.05)。结论前交叉韧带重建术后予以患者早期康复训练联合神经肌肉电刺激可以更好地预防腘绳肌肌力下降与电机械时间延长,改善膝关节功能。Objective To investigate the effect of neuromuscular electrical stimulation combined with early rehabilitation training on hamstring function after anterior cruciate ligament reconstruction.Methods From January 2018 to December 2020,70 patients with anterior cruciate ligament rupture admitted to the Department of Rehabilitation Medicine of the First Affiliated Hospital of Fujian Medical University were conveniently selected as the research object.All patients were taken from their own hamstrings for anterior cruciate ligament reconstruction,and they were randomly divided into two groups.The 35 cases in the control group received early hamstring rehabilitation training after the operation;the 35 cases in the study group received neuromuscular electrical stimulation on the basis of the control group.After the intervention,the bilateral hamstring isometric muscle strength and electromechanical delay of the two groups were measured,and the knee joint function of the two groups was evaluated by the IKDC scale.Results After intervention,the study group's hamstring muscle strength(31.95±4.82)N·m/kg and IKDC score(61.77±4.38)points were higher than those of the control group,and the difference was statistically significant(t=4.280,6.802,P<0.05).The electromechanical delay of the affected side semitendinosus muscle(27.45±2.23)ms and the electrome chanical delay of biceps femoris(29.74±3.06)ms after intervention in the study group were lower than those of the control group,and the difference was statistically significant(t=5.292,8.916,P<0.05).Conclusion After ACL reconstruction,early rehabilitation training combined with neuromuscular electrical stimulation can better prevent hamstring muscle strength decline and electromechanical time extension,and improve knee joint function.
