摘 要:为对比分析不同检测方法在沙眼衣原体感染中的应用效果与价值,本文选取2020年1月—12月在我院就诊的949名女性为研究对象;采集其宫颈分泌物949份,分别行胶体金法与FQ-PCR法(实时荧光定量PCR法)检测,对比分析不同检测方法的结果。结果显示,在949份样本中,经细胞培养检出阳性93例、阴性856例;经FQ-PCR法检出阳性91例、阴性858例;胶体金法检出阳性50例、阴性899例。参照金标准结果可见,FQ-PCR法检测的阳性检出率、诊断符合率、检验灵敏度、阳性预测值、阴性预测值及诊断特异度分别为9.59%、96.84%、82.80%、84.62%、98.14%、98.36%,均高于胶体金法检测结果的5.27%、89.15%、21.51%、40.00%、91.88%、96.50%,(P<0.05)。由此可见,在沙眼衣原体感染检测中,FQ-PCR法具有较高的准确性,且检测速度较快,适用于沙眼衣原体的临床检测,但其对实验室条件和检测人员水平的要求较高;胶体金法虽然诊断灵敏度、特异度低于FQ-PCR法,但其操作相对简单,检测速度快,且对实验室条件、检测仪器均无特殊要求,适用于基层医院。故在沙眼衣原体感染检测的实际应用中,应当结合医院实际情况对检测方案进行合理选择,可以FQ-PCR法为首选检测方法,并以胶体金法为辅助检测方法,以促进疾病诊断准确性的提升。In order to compare and analyze the application effect and value of different detection methods in Chlamydia trachomatis infection,949 female in our hospital from January to December 2020 were selected as the research object;949 cervical secretions were collected and detected by colloidal gold method and FQ-PCR(real-time fluorescence quantitative PCR). The results of different detection methods were compared and analyzed. The results showed that 93 cases were positive and 856 cases were negative in 949 samples;91 cases were positive and 858 cases were negative by FQ-PCR;Colloidal gold method was positive in 50 cases and negative in 899 cases. Referring to the results of gold standard,the positive detection rate,diagnostic coincidence rate,test sensitivity,positive predictive value,negative predictive value and diagnostic specificity of FQ-PCR were 9.59%,96.84%,82.80%,84.62%,98.14% and98.36% respectively,which were higher than 5.27%,89.15%,21.51%,40.00%,91.88% and 96.50% of the results of colloidal gold method(P < 0.05). It can be seen that FQ-PCR has high accuracy and fast detection speed in the detection of Chlamydia trachomatis infection. It is suitable for the clinical detection of Chlamydia trachomatis,but it has high requirements for laboratory conditions and the level of detection personnel;Although the diagnostic sensitivity and specificity of colloidal gold method are lower than FQ-PCR method,its operation is relatively simple,the detection speed is fast,and there are no special requirements for laboratory conditions and detection instruments. It is suitable for grass-roots hospitals. Therefore,in the practical application of Chlamydia trachomatis infection detection,the detection scheme should be reasonably selected in combination with the actual situation of the hospital. FQ-PCR method can be the preferred detection method,and colloidal gold method can be used as an auxiliary detection method,so as to improve the accuracy of disease diagnosis.
