地榆升白片改善高龄晚期胃癌患者口服化疗中性粒细胞减少的临床观察

摘　　要：目的评估地榆升白片治疗高龄晚期胃癌患者口服化疗中性粒细胞减少的临床效果。方法收集南京市第一医院肿瘤内科2014—2018年70~80岁晚期胃癌患者100例,随机分为观察组和对照组,各50例,观察组口服化疗期间同步服用地榆升白片,对照组单纯口服化疗,比较分析两组患者的首次中性粒细胞减少时间、中性粒细胞减少程度、中性粒细胞减少致化疗中断时间以及治疗前后两组内源性粒细胞集落刺激因子(granulocyte colony stimulating factor,G-CSF)水平、免疫功能指标CD4^(+)/CD8^(+)变化。结果与对照组比较,观察组化疗后首次中性粒细胞减少时间明显延长,中性粒细胞减少致化疗中断天数明显缩短,差异均有统计学意义(P=0.021;P=0.028)。首次中性粒细胞减少患者中,观察组Ⅰ度患者比例明显高于对照组,治疗后观察组内源性G-CSF水平、CD4^(+)/CD8^(+)水平显著高于对照组。结论地榆升白片治疗可显著改善高龄晚期胃癌患者口服化疗诱导的中性粒细胞减少,疗效安全,值得大力推广。Objective To evaluate the clinical effect of Diyu Shengbai tablets on neutropenia in elderly patients with advanced gastric cancer.Methods Totally 100 patients with advanced gastric cancer from 70 to 80 years old in Nanjing First Hospital from 2014 to 2018 were involved and randomly divided into an observation group(n=50)and a control group(n=50).The patients received Diyu Shengbai tablets during the oral chemotherapy in the observation group,while the control group only received oral chemotherapy drugs.The occurrence of the first neutropenia,the degree of neutropenia and the length of chemotherapy interruption caused by neutropenia were compared between the two groups.The serum level of endogenous granulocyte colony stimulating factor(G-CSF)and the ratios of CD4^(+)/CD8^(+)were detected in both groups before and after the chemotherapy.Results Compared with the control group,the occurrence of the first neutropenia was significantly prolonged in the observation group,and the number of days of chemotherapy interruption caused by neutropenia was reduced obviously.The proportion of patients with degreeⅠnetropenia in the observation group was higher than that in the control group.The serum level of endogenous G-CSF and the ratio of CD4^(+)/CD8^(+)were also significantly higher in the observation group.Conclusion Diyu Shengbai tablets plays an active role in improving the neutropenia among elderly patients with advanced gastric cancer in oral chemotherapy.
