摘  要:目的探讨结构-过程-结果三维质量评价模式在小儿肠造口出院延续护理中的应用效果。方法选取湖南省儿童医院2017年5月—2019年5月收治的80例肠造口患儿及其家长80名,采用随机数字表法分为对照组与观察组,各40例患儿及其家长40名。对照组实施常规出院护理,观察组实施基于结构-过程-结果三维质量评价模式的出院延续护理。分别于护理前和出院后1、3个月采用自制造口照护健康知识调查表评估家长的造口护理健康知识,采用家庭照顾者能力测量量表(FCTI)评价家长的照顾能力。结果出院后3个月,两组肠造口患儿家长的造口基础知识评分与护理技能评分均升高,组内比较差异均有统计学意义(P<0.05);出院后1、3个月,观察组患儿家长造口基础知识、护理技能评分均高于对照组,FCTI评分低于对照组,差异均有统计学意义(P<0.05)。观察组患儿的造口相关并发症发生率低于对照组,家长护理满意度高于对照组,差异均有统计学意义(P<0.05)。结论基于结构-过程-结果三维质量评价模式对肠造口患儿实施出院延续护理能够提高家长造口护理技能与照顾能力,降低患儿造口相关并发症,值得临床推广。Objective To explore the effect of the structure-process-outcome three-dimensional quality evaluation model in the transitional care of discharged children with enterostomy.Methods A total of 80 children with enterostomy and their 80 parents in the Hunan Children's Hospital were selected from May 2017 to May 2019.Patients were divided into the control group and the observation group,each with 40 children and their 40 parents according to the random number table method.The control group performed routine discharge nursing,and the observation group carried out discharge transitional care based on the structure-process-outcome three-dimensional quality evaluation model.The self-made Oral Care Health Knowledge Questionnaire was used to assess the health knowledge of the parents'stoma care,and the Family Caregiver Task Inventory(FCTI)was used to evaluate the care ability of the parents before nursing and one and three months after discharge.Results Three months after discharge,the scores of basic stoma knowledge and nursing skills of the parents of the two groups of children with enterostomy increased,and the differences within the group were statistically significant(P<0.05).The scores of basic stoma knowledge and nursing skills of the parents of the observation group were higher than those of the control group,and the scores of FCTI were lower than those of the control group one and three months after discharge,and the differences were statistically significant(P<0.05).The incidence of stoma-related complications of children in the observation group was lower than that of the control group,and parental care satisfaction was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusions Based on the structure-process-outcome three-dimensional quality evaluation model,the implementation of transitional care for children with enterostomy can improve the nursing skills and care ability of parents,and reduce the stoma-related complications in children,and it is worthy of clin
