摘  要:目的探究血管回声跟踪(ET)技术定量评价老年冠心病(CHD)患者颈总动脉弹性及与心肌质量指数(LVMI)相关性。方法选取2017年5月至2019年5月我院收治的100例老年CHD患者作为观察组,同期纳入97例老年CHD高危人群作为对照组,均接受ET检查。统计两组患者的颈总动脉弹性指标[僵硬度(β)、压力应变弹性系数(Ep)、顺应性(AC)]、超声心动图参数[心肌质量指数、左室舒张末期内径(LVEDd)、左心室后壁厚度(LVPWd)],分析颈总动脉弹性指标与超声心动图参数相关性及二者联合诊断效能。结果观察组AC低于对照组,Ep、β高于对照组(P<0.05);老年CHD患者颈总动脉弹性指标AC与LVMI呈负相关,Ep、β与LVMI呈正相关(P<0.05);颈总动脉弹性指标及超声心动图参数联合诊断老年CHD患者颈总动脉弹性的AUC(0.845)>AC(0.785)>β(0.763)>LVMI(0.754)>LVEDd(0.737)>LVPWd(0.708)>Ep(0.678)。结论AC、Ep、β参数与心肌质量指数存在一定关联性,ET技术定量评价联合超声心动图参数有望成为诊断老年CHD患者颈总动脉弹性的重要手段。Objective To investigate the elasticity of common carotid artery as quantitatively assessed by echo tracking(ET)and its correlation with left ventricular mass index(LVMI)in elderly patients with coronary heart disease(CHD).Methods A total of 100 elderly CHD patients admitted to our hospital between May 2017 and May 2019 were enrolled as the stuy group.A cohort of 97 elderly subjects who were at high-risk for CHD were enrolled as the control group.The both groups underwent ET assessment,and were recorded for parameters of common carotid artery elasticity[vessel stiffness(β),pressure strain elastic modulus(Ep),and arterial compliance(AC)]and echocardiography[LVMI,left ventricular end-diastolic diameter(LVEDd),and diastolic left ventricular posterior wall thickness(LVPWd)].The correlation between common carotid artery elasticity and echocardiographic parameters was analyzed,as well as the diagnostic efficiency when the both were used in combination.Results The study group had lower AC,higher Ep andβ,as compared with the control group(P<0.05).Of the common carotid artery elasticity parameters in elderly CHD patients,AC was negatively correlated with LVMI,and Ep andβwere positively correlated with LVMI(P<0.05).The area under curve(AUC)for assessing common carotid artery elasticity in elderly CHD patients was greatest with combined use of common carotid artery elasticity and echocardiographic parameters(0.845),followed by AC(0.785),β(0.763),LVMI(0.754),LVEDd(0.737),LVPWd(0.708),and Ep(0.678).Conclusion AC,Ep andβas common carotid artery elasticity parameters showed certain correlation with LVMI.Combining quantitative ET findings and echocardiographic parameters appears promising as an important tool for assessing common carotid artery elasticity in elderly CHD patients.
