摘  要:目的探讨甲状腺癌患者切除组织中环氧合酶-2(COX-2)、M2型巨噬细胞标志物CD163、血管内皮生长因子(VEGF)的表达及其临床意义。方法回顾性分析我院2016年6月至2017年6月首诊并治疗的113例甲状腺癌患者资料,纳入甲状腺癌组,另选取同期手术切除的甲状腺瘤组织74例为甲状腺瘤组,正常甲状腺组织61例为对照组。所有研究对象甲状腺组织均经免疫组化法检测COX-2、CD163、VEGF表达水平。分析COX-2、CD163、VEGF表达与甲状腺癌临床病理特征的关系,采用Kaplan-Meier生存曲线分析COX-2、CD163、VEGF表达对3年总生存率的影响。结果甲状腺癌组组织中COX-2、CD163、VEGF阳性表达率明显高于甲状腺瘤组、对照组(P<0.05),甲状腺瘤组组织中COX-2、CD163、VEGF阳性表达率明显高于对照组(P<0.05);不同病理类型、临床分期、淋巴结转移情况、肿瘤胞膜情况甲状腺癌COX-2、CD163、VEGF阳性表达率均具有显著差异(P<0.05);COX-2阳性表达甲状腺癌总生存率显著低于阴性表达(P<0.05),CD163阳性表达甲状腺癌总生存率显著低于阴性表达(P<0.05),VEGF阳性表达甲状腺癌总生存率显著低于阴性表达(P<0.05)。结论甲状腺癌患者切除组织中COX-2、CD163、VEGF阳性表达率高,其阳性表达与病理类型、临床分期、淋巴结转移、胞膜情况及预后密切相关。Objective To investigate the expression and clinical significance of cyclooxygenase-2(COX-2),M2 macrophages marker CD163 and vascular endothelial growth factor(VEGF)in resected tissues from patients with thyroid cancer.Methods A total of 113 patients with initial diagnosis and treatment of thyroid cancer in our hospital between June 2016 to June 2017 were retrospectively enrolled and assigned to the thyroid cancer group.A contemporary cohort of 74 patients with thyroid adenoma and 61 others with normal thyroid function were assigned to the thyroid adenoma group and control group,respectively.The expression levels of COX-2,CD163 and VEGF in thyroid tissues were detected by immunohistochemistry.The relationship between the COX-2,CD163,and VEGF expression and clinicopathological features of thyroid carcinoma was analyzed.Kaplan Meier survival curve was used to analyze the impact of COX-2,CD163,and VEGF expression on 3-year overall survival rate.Results Among three groups,the positive expression rates of COX-2,CD163 and VEGF were the highest in the thyroid cancer group,followed by thyroid adenoma group,and were lowest in the control group(all P<0.05).The positive expression rates of COX-2,CD163 and VEGF were significantly higher in the thyroid adenoma group than those in the control group(P<0.05).Of thyroid cancer tissues,the expression rates of COX-2,CD163 and VEGF varied significantly according to pathological types,clinical stages,lymph node metastasis and tumor capsule(P<0.05).The overall survival rate was significantly lower in thyroid cancer patients with positive COX-2 expression than those with negative expression(P<0.05),as were in patients with positive CD163 expression than those with negative expression(P<0.05),and in patients with positive VEGF expression than those with negative expression(P<0.05).Conclusion The positive expression rates of COX-2,CD163 and VEGF are high in resected tissues from thyroid cancer patients.The positive expression of these markers is closely related to pathological types,clin
