摘　　要：目的分析老年膝关节炎患者介入TTM模型的护理策略对其康复活动执行度的影响。方法选取北京积水潭医院2016年12月~2017年11月收治的老年膝关节炎患者47例为对照组,实施传统护理干预;2017年12月〜2018年11月收治的膝关节炎患者47例为研究组,开展TTM模型的护理策略。比较两组不良事件的发生率;比较两组在干预前与干预后不同行为阶段的人数情况;对两组运动锻炼的完成情况进行评分;利用自制的满意度问卷对两组满意度情况进行测评。结果两组不良事件发生率差异无统计学意义(P>0.05);研究组干预后所处的行动阶段与维持阶段人数多于对照组,差异有统计学意义(P<0.05);研究组依从行为评分高于对照组,差异有统计学意义(P<0.05);研究组护理满意度高于对照组,差异有统计学意义(P<0.05)。结论介入TTM模型的护理策略应用于老年膝关节炎患者,能提高患者的内在行为改变动机,利于其主动参与到家庭康复活动的执行进程中来。Objective To analyze the impact of transtheoretical model(TTM)-based nursing strategy on the implementation of the rehabilitation activities in older patients with knee arthritis.Methods A total of 47 older patients with knee arthritis treated with conventional intervention in Beijing Jishuitan Hospital from December 2016 to Novermber 2017 were assigned into the control group.47 patients with knee arthritis treated with TTM-based intervention from December 2017 to Novermber 2018 were assigned into the research group.The incidence of adverse events and the number of patients in different behavioral stages before and after intervention were compared between the two groups.The implementation of exercise and the satisfaction degree of patients in the two groups were assessed.Results There was no significant difference in the incidence of adverse events between the two groups(P>0.05).The number of patients in the action stage and maintenance stage after intervention in the research group were higher than those in the control group(P<0.05).The compliance score in the research group was higher than that in the control group(P<0.05).The satisfaction degree in the research group was higher than that in the control group(P<0.05).Conclusion TTM-based nursing strategy might enhance the intrinsic motivation of older patients with knee arthritis to change behavior,and facilitate their active participation in the implementation process of family rehabilitation activities.
