摘 要:目的进一步确证呼吸道感染常见细菌分泌结核分枝杆菌MPT64蛋白或抗原类似物的情况,为结核病试验诊断研究提供循证依据。方法选取2019年3月至2020年12月清远市清新区人民医院送检的合格痰标本80株培养分离株,依细菌分离时间先后将样本分为训练集样本组50株和测试集样本组30株。取ATCC25923金黄色葡萄球菌、ATCC29213金黄色葡萄球菌、ATCC25922大肠埃希菌、ATCC35218大肠埃希菌、ATCC27853铜绿假单胞菌、ATCC29212粪肠球菌、ATCC1766嗜麦芽窄食单胞菌等标准参考株和肺炎克雷伯菌、铜绿假单胞菌、嗜麦芽窄食单胞菌、大肠埃希菌、金黄色葡萄球菌、鲍曼不动杆菌、表皮葡萄球菌、奇异变形杆菌、阪崎肠杆菌、阴沟肠杆菌等80株痰样本临床分离株之血平板培养菌落生理盐水冲刷液,应用胶体金免疫法检测冲刷液MPT64。应用频数分布分别对训练集样本、测试集样本和全部检测样本进行统计学描述,计算其MPT64阳性检出率。结果在7株标准参考株中,仅ATCC29212粪肠球菌菌落生理盐水冲刷液MPT64检测阳性;在80株临床分离株中,肺炎克雷伯菌、铜绿假单胞菌、嗜麦芽窄食单胞菌、金黄色葡萄球菌菌落生理盐水冲刷液MPT64检测阳性率分别为24.0%(6/25)、15.0%(3/20)、35.7%(5/14)、40.0%(2/5),9株大肠埃希菌、3株鲍曼不动杆菌和各1株的表皮葡萄球菌、奇异变形杆菌、阪崎肠杆菌、阴沟肠杆菌之冲刷液MPT64检测阴性,痰液常见细菌冲刷液MPT64检测总体阳性率为20.0%(16/80)。结论不少常见呼吸道感染性细菌菌种可致MPT64检测阳性,在结核病诊疗中直接检测呼吸道样本MPT64时,宜结合其普通细菌检测结果进行综合分析。Objective To confirm further common bacteria of respiratory tract infection that may secrete MPT64,a special secretion protein of M.tuberculosis,or its antigenic analog,and to provide evidence-based basis for tuberculosis experiment diagnosis.Methods Eighty qualified sputum samples from Qingxin District People's Hospital from March 2019 to December 2020 were selected for culture isolates.According to the bacterial separation time,the samples were divided into a training set sample group(50 strains)and a test set sample group(30 strains).Seven reference strains,including S.aureus(ATCC25923),S.aureus(ATCC29213),E.coli(ATCC25922),E.coli(ATCC35218),P.aeruginosa(ATCC27853),E.faecalis(ATCC29212),and S.maltophilia(ATCC1766),and eighty clinical separation strains from sputum,including K.peumoniae,P.aeruginosa,S.maltophilia,E.coli,S.aureus,A.baumannii,S.epidermidis,P.mirabilis,E.sakazakii,and E.cloacae,were selected as research objects.Their MPT64 in normal saline rinsing fluid of bacteria colony separated from blood culture were detected by the colloidal gold method.The frequency distribution was used to statistically describe the training set sample,test set sample,and all test samples,and the MPT64 positive detection rate was calculated.Results MPT64 was positive in normal saline rinsing fluid of E.faecalis(ATCC29212)among the seven reference strains.Of eighty clinical separation strains,MPT64 was positive in normal saline rinsing fluid of K.peumoniae,P.aeruginosa,S.maltophilia,and S.aureus,and their positive rates were 24.0%(6/25),15.0%(3/20),35.7%(5/14),and 40.0%(2/5),respectively.MPT64 from the other strains,including E.coli(9 strains),A.baumannii(3 strains),S.epidermidis(1 strain),P.mirabilis(1 strain),E.sakazakii(1 strain),and E.cloacae(1 strain),were all negative.The overall positive rate of MPT64 was 20.0%(16/80).Conclusions The common bacteria of respiratory tract infection can cause MPT64 detection positive.It is essential that preferred to combine their ordinary bacterial test results for tuberculosis diagnos
