摘　　要：目的探讨纤维支气管镜下支气管肺泡灌洗(BAL)联合亚胺培南-西司他丁钠治疗对重症肺部感染的疗效影响。方法108例重症肺部感染患者,根据不同治疗方式分为研究组(n=55,采用BAL+亚胺培南-西司他丁钠治疗),对照组(n=53,采用亚胺培南-西司他丁钠治疗)。对比两组临床疗效、肺功能、炎性因子、氧化应激及不良反应。结果研究组总疗效高于对照组(P<0.05);治疗后研究组1秒用力呼气容器(FEV_(1))、用力肺活量(FVC)、1秒用力呼气容积占用力肺活量的百分比(FEV_(1)/FVC%)、超氧化物歧化酶高于对照组,肿瘤坏死因子-α、C-反应蛋白、白介素-8、丙二醛低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论纤维支气管镜下BAL联合亚胺培南-西司他丁钠治疗治疗重症肺部感染临床疗效更佳,且不增加不良反应。Objective To investigate the effect of bronchoalveolar lavage(BAL)+imipenem-cilastatin sodium treatment on curative effect of severe pulmonary infection under fiberoptic bronchoscope.Methods One hundred and eight patients with severe lung infection admitted to our hospital were divided into a study group(n=55)and a control group(n=53)according to different treatment methods.The study group was treated with BAL+imipenem-cilastatin sodium while the control group was treated with imipenem-cilastatin sodium.The clinical efficacy,lung function,inflammatory factors,oxidative stress and adverse reactions were compared between the two groups.Results The total curative effect of the study group was higher than that of the control group(P<0.05).After treatment,forced expiratory volume in 1 second(FEV_(1)),fast vital capacity(FVC),FEV_(1)/FVC%,superoxide dismutase(SOD)were higher while tumor necrosis factorα(TNF-α),C-reactive protein(CRP),interleukin 8(IL-8)and malondialdehyde(MDA)were lower in the study group than those in the control group(P<0.05).The difference in the incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion Bronchoalveolar lavage under fiberoptic bronchoscopycombined with imipenem-cilastatin sodium has a better clinical effect in the treatment of severe pulmonary infection without increasing adverse reactions.
