甲孕酮联合克林霉素磷酸酯治疗子宫内膜炎临床疗效及可行性研究

摘　　要：目的探讨甲孕酮联合克林霉素磷酸酯治疗子宫内膜炎的临床疗效及可行性。方法2019年3月至2021年1月在我院治疗的127例子宫内膜炎患者,根据治疗方案不同分为对照组66例(克林霉素磷酸酯治疗)与观察组61例(克林霉素磷酸酯+甲孕酮治疗),均治疗3个疗程,观察两组疗效、临床指标、炎性因子水平及免疫指标水平改善情况,分析用药安全性。结果对照组总有效率低于观察组(P<0.05);对照组腹痛消失时间、炎症消失时间、体温恢复正常时间及住院时间均长于观察组(P<0.05);治疗后,两组IL-4、IL-6、C反应蛋白、肿瘤坏死因子水平均下降,CD4水平升高,CD38、CD138水平均下降,且观察组各指标优于对照组(P<0.05);两组不良反应总发生率比较差异无统计学意义(P>0.05)。结论甲孕酮联合克林霉素磷酸酯治疗子宫内膜炎患者安全性高,且可有效抑制机体炎性水平、改善免疫功能,临床疗效显著,临床应用前景广阔。Objective To explore the clinical efficacy and feasibility of medroxyprogesterone acetate combined with clindamycin phosphate in the treatment of endometritis.Methods One hundred and twenty-seven patients with endometritis treated in our hospital from March 2019 to January 2021 were divided into a control group(n=66)and an observation group(n=61).The control group was treated with clindamycin phosphate and the observe group was treated with clindamycin phosphate+medroxyprogesterone acetate.Both groups were treated for 3 courses.The therapeutic effects,the clinical indicators,the levels of inflammatory factors and the improvement of immune indicators in the two groups were observed,and the safety of the drugs was analyzed.Results The total effective rate of the control group was lower than that of the observation group(P<0.05).The disappearance time of abdominal pain,the disappearance time of inflammation,the time for body temperature returning to normal,and the length of hospital stay in the control group were longer than those in the observation group(P<0.05).After treatment,the levels of interleukin-4(IL-4),IL-6,C-reactive protein(CRP),and tumor necrosis factors-α(TNF-α)in the two groups were decreased,the level of CD4 was increased while the levels of CD38 and CD138 were decreased,and the observation group was better than the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Medroxyprogesterone acetate combined with clindamycin phosphate in the treatment of endometritis has a high safety.The drugs can effectively inhibit the body′s inflammatory level and improve the immune function.The clinical effect is significant.The clinical application prospects are broad.
