摘 要:目的探讨微创后路减压椎弓根螺钉钉道强化内固定治疗腰椎骨折的临床疗效。方法选取2019年6月至2021年1月我院收治的腰椎骨折中老年患者60例,根据手术方式不同分组,对照组30例采用传统开放式椎弓根螺钉内固定术,研究组30例采用微创后路减压联合椎弓根螺钉钉道强化内固定术,比较两组治疗相关情况、手术前后VAS疼痛评分及Cobb角、手术前后Frankel神经功能分级变化。结果研究组术中时间、术中出血量及住院时间等显著优于对照组(P<0.05);治疗后VAS疼痛评分及Cobb角明显小于对照组(P<0.05);术后随访,研究组Frankel神经功能分级优于对照组(P<0.05)。结论在中老年腰椎骨折治疗中,微创后路减压椎弓根螺钉钉道强化内固定临床疗效显著,值得临床借鉴。Objective To investigate the clinical effect of minimally invasive posterior decompression and enhanced internal fixation with pedicle screws in the treatment of lumbar fracture.Methods Sixty middle-aged and elderly patients with lumbar fractures admitted to our hospital from June 2019 to January 2021 were selected.According to the surgical methods,the patients were divided into a control group(n=30)treated with conventional open pedicle screw internal fixation,and a study group(n=30)treated with minimally invasive posterior decompression combined with enhanced internal fixation with pedicle screws.Treatment-related conditions,VAS pain scores and Cobb horns,and Frankel neurological functiongrade changes before and after surgery were compared between the two groups.Results The intraoperative time,intraoperative bleeding,and hospital stay in the study group were significantly better than those in the control group(P<0.05).After treatment,VAS pain scores and Cobb horns in the study group were significantly smaller than those in the control group(P<0.05).During follow-up,the study group had a better Frankel neurological function classification than the control group did(P<0.05).Conclusion In the treatment of lumbar fracture in middle-aged and elderlypatients,minimally invasive posterior decompression and enhanced internal fixation with pedicle screws is remarkable.It is worthy of clinical reference.
