摘　　要：目的研究CO_(2)点阵激光联合环钻术在剖宫产术后疤痕修复中的应用效果,为临床治疗提供指导。方法选取2019年1月至2021年1月大连市皮肤病医院收治的94例剖宫产皮肤瘢痕患者进行研究,按照随机数表法分为观察组和对照组各47例。对照组患者应用CO_(2)点阵激光修复,观察组患者应用CO_(2)点阵激光联合环钻术修复。分别于治前、治疗后3个月、6个月、9个月观察并比较两组患者的温哥华瘢痕评分(VSS)和美观满意度[采用视觉模拟评分法(VAS)评价],于治疗后9个月评估患者临床治疗效果及并发症发生情况。结果治疗后,观察组患者的治疗总有效率为91.49%,明显高于对照组的68.09%,差异具有统计学意义(P<0.05);治疗后3个月、6个月、9个月,观察组患者的VSS评分分别为(5.44±1.35)分、(4.44±1.58)分、(3.91±1.04)分,明显低于对照组的(6.47±1.74)分、(5.37±1.91)分、(4.51±1.35)分,差异均有统计学意义(P<0.05);治疗后3个月、6个月、9个月,观察组患者的美观满意度评分分别为(5.21±0.24)分、(7.22±0.44)分、(9.52±0.75)分,明显高于对照组的(4.14±0.17)分、(5.52±0.37)分、(7.12±0.45)分,差异均具有统计学意义(P<0.05);观察组患者的并发症发生率为10.64%,明显低于对照组的31.91%,差异有统计学意义(P<0.05)。结论CO_(2)点阵激光联合环钻术在剖宫产术后疤痕修复中的应用效果较单用CO_(2)点阵激光的效果更为显著,其不仅能有效提高治疗效果和美观满意度,也无明显并发症,值得临床推广应用。Objective To study the application effect of CO_(2)fractional laser combined with trephine in scar repair after cesarean section, and to provide guidance for clinical research. Methods A total of 94 patients with cesarean section skin scars admitted to Dalian Dermatosis Hospital from January 2019 to January 2021 were selected for the study. According to the random number table, they were divided into an observation group and a control group, with 47 patients in each group. The control group was repaired by CO_(2)fractional laser, and the observation group was repaired by CO_(2)fractional laser combined with trephine. The Vancouver Scar Scale(VSS) scores and aesthetic satisfaction evaluated by the visual analogue scale(VAS) between the two groups were observed and compared before treatment and 3, 6,and 9 months after treatment. The clinical treatment effects and complications of the patients were evaluated 9 months after treatment. Results After treatment, the total effective rate of treatment in the observation group was 91.49%, which was significantly higher than 68.09% in the control group(P<0.05). At 3 months, 6 months, and 9 months after treatment, the VSS scores of observation group were(5.44±1.35) points,(4.44±1.58) points, and(3.91±1.04) points, which were significantly lower than(6.47±1.74) points,(5.37±1.91) points,(4.51±1.35) points of the control group(P<0.05).At 3 months, 6 months, and 9 months after treatment, the aesthetic satisfaction scores of the observation group were(5.21±0.24) points,(7.22±0.44) points, and(9.52±0.75) points, which were significantly higher than(4.14±0.17) points,(5.52±0.37) points,(7.12±0.45) points of the control group(P<0.05). The complication rate of the observation group was10.64%, which was significantly lower than 31.91% of the control group(P<0.05). Conclusion The application effect of CO_(2)fractional laser combined with trephine in scar repair after cesarean section is more significant than that of CO_(2)fractional laser alone.
