摘 要:目的探讨极低出生体质量(VLBW)早产儿血清磷钙、碱性磷酸酶(ALP)及25羟维生素D_(3)[25-(OH)D_(3)]的变化及其临床意义。方法选取2018年1月至2020年12月兵器工业五二一医院新生儿科收治的40例VLBW早产儿纳入病例组,另收集出生日龄与病例组相近的同期足月儿40例作为对照组,均分别于出生后1周、4周及8周采集静脉血,检测并比较两组新生儿的血清磷钙、ALP及25-(OH)D_(3)水平。结果病例组早产儿出生后1周、4周及8周时血清ALP水平依次为(862.65±178.58) U/L、(916.84±165.87) U/L、(933.58±172.44) U/L,明显高于对照组的(496.63±134.74) U/L、(512.41±142.78) U/L、(508.68±136.58) U/L,血清25-(OH)D_(3)水平依次为(15.68±5.13) mmol/L、(17.14±4.67) mmol/L、(18.96±5.23) mmol/L,明显低于对照组的(26.71±5.42) mmol/L、(25.73±5.38) mmol/L、(27.36±4.98) mmol/L,差异均有统计学意义(P<0.05);而两组新生儿各时间点的血钙、磷水平比较差异均无统计学意义(P>0.05)。结论 VLBW早产儿血清ALP、25-(OH)D_(3)水平与足月产儿存在明显差异,提示动态监测这二项指标对于VLBW早产儿代谢性骨病早期发现与及时干预有重要指导意义。Objective To investigate the changes and clinical significance of serum phosphorus and calcium,alkaline phosphatase (ALP) and 25-hydroxyvitamin D_(3)[25-(OH)D_(3)]in premature infants with very low birth weight(VLBW).Methods Forty premature infants with VLBW,who admitted to Department of Neonatology,521 Hospital of Norinco Group from January 2018 to December 2020 were selected as the case group.Meanwhile,40 full-term infants of similar age were selected as the control group.Venous blood was collected at 1 week,4 weeks and 8 weeks after birth to detect the levels of serum phosphorus and calcium,ALP and 25-(OH)D_(3)in the two groups of neonates.Results The serum ALP levels of premature infants in the case group at 1 week,4 weeks and 8 weeks after birth were (862.65±178.58) U/L,(916.84±165.87) U/L and (933.58±172.44) U/L,respectively,which were significantly higher than corresponding (496.63±134.74) U/L,(512.41±142.78) U/L and (508.68±136.58) U/L of the control group (all P<0.05).Serum25-(OH)D_(3)levels were (15.68±5.13) mmol/L,(17.14±4.67) mmol/L and (18.96±5.23) mmol/L,which were significantly lower than corresponding (26.71±5.42) mmol/L,(25.73±5.38) mmol/L and (27.36±4.98) mmol/L of the control group(all P<0.05).There was no significant difference in serum calcium or phosphorus levels between the two groups of neonates at different time points (P>0.05).Conclusion The levels of serum ALP and 25-(OH)D_(3)in premature infants with VLBW are significantly different from those in full-term infants,suggesting that monitoring the two indicators is of great significance for early detection of metabolic bone disease in premature infants with VLBW and timely intervention.
